World review of laparoscopic treatment of liver cystic echinococcosis—914 patients

Abstract: The laparoscopic approach is safe with acceptable mortality and morbidity for both conservative and radical resections in selected patients. Clinical outcomes are comparable to open surgery, albeit in a selected group of patients.

“…These procedures demonstrate the risk of hydatid dissemination and problems stemming from the formation of residual cavities, a conversion percentage of up to 30 per cent; higher POM between 0 and 54 per cent, which could be associated with inadequate patient selection; average hospitalization between 1 and 8 days; and recurrence between 0 and 11 per cent. These data are presented in a systematic review of nonrandomized studies, which summarizes the outcomes of 914 patients treated by this type of access, taken from 57 primary articles (evidence type 3a) . Despite the potential problems that this alternative provides us, there is a series of advantages that must be taken into consideration, such as the possibility of inspecting the residual cavity in search of biliary communications (Fig.…”

“…The indications have expanded through the years. The only excluding criteria for minimally invasive intervention are deep intra-parenchymal cysts, posterior cysts situated close to the vena cava, more than 5 cysts, multilocular cysts with thick and calcified walls 23. According to our institutional protocol, laparoscopy was offered to patients diagnosed with single superficial lesions measuring less than 5 cm, without suspicion of cysto-biliary communication.…”

“…The current data, that resulting from large series and reviews, suggests that the aparoscopic approach is safe in selected patients, but is unclear whether the laparoscopic approach will ever surpass the traditional open approach and become the gold standard of treatment for selected hyadatid cysts. 2 Most authors seem to agree that the size of the hydatid cyst should not influence or limit the indication for the laparoscopic approach. The exclusion criteria were cysts located in segments 1 or 7 of the liver and evidence of intrabiliary rupture.…”
