Background: Depression and coronary heart disease (CHD) often occur together in clinical practice. As a traditional Chinese medicine, Kai-Xin-San (KXS) has been widely used for the treatment of emotion-related disorders. In the present study, we aimed to explore whether KXS had both antidepressive effects and cardioprotective functions in a rat model of myocardial ischemia (MI) with depression. Methods: A total of 50 SD rats were randomly assigned into five groups as follows: normal control (control group), celiac injection of isopropyl adrenaline (ISO) (MI group), depression (depression group), MI+ depression (model group) and MI+ depression treated with intragastric administration of 370 mg/kg KXS (KXS group). MI was induced by subcutaneous injection of 85 mg/kg ISO. Depression was developed by a 7-week chronic mild stress (CMS) challenge. Behavioral test was conducted before and during the experiment. Echocardiography and biochemical analysis were carried out after 7 weeks of CMS challenge. Results: After 7 weeks of experiment, depression-like behaviors were observed in all the groups except for control and KXS groups, and KXS treatment dramatically increased open-field test scores and sucrose consumption (P < 0.01 vs. model group). Echocardiography and biochemical analysis showed that KXS treatment could improve levels of ejection fraction (EF) and fractional shortening (FS), which were reduced by depression and ISO challenge. Meanwhile, KXS treatment significantly decreased the levels of creation kinase MB (CK-MB) and lactate dehydrogenase (LDH), which were increased in the model group. The activities of superoxide dismutase (SOD), glutathione peroxidase (GSH-PX), catalase (CAT) were increased, while the malondialdehyde (MDA) activity was significantly decreased in the KXS group. Although acupuncture therapy, as a preventive and therapeutic technique, is low or no risk for some cardiovascular diseases, its role in safeguarding myocardial hypertrophy (MH) has not been clarified. Therefore, this study focused on examining whether electroacupuncture (EA) can alleviate MH by up-regulating tryptophan 5-serotonin (5-HT) metabolism, depressive disorder, and gastrointestinal dysfunction in MH rat models. We evaluated the effect of EA on myocardial ischemia in the ISO-induced myocardial hypertrophy (MH) rat model. Forty Sprague–Dawley (SD) rats aged 3 months were randomly divided into normal group, model group, EA group, and sham EA group with 10 rats in each group. Heart weight/body weight ratio (HW/BW) and left ventricular weight/body weight ratio (LVW/BW) were used to evaluate the regression of MH. Cardiac pathology was evaluated by ultrastructural observation and hematoxylin and eosin (H&E) staining. Sucrose preference test (SPT), forced swimming test (FSW), and open-air test (OFT) were used to study depressive behavior. The gastric retention and intestinal transport were evaluated by carbon powder labeling and the expression of 5-HT in the cerebral cortex and ileum was detected by immunohistochemistry. Depression-like symptoms in rats can be relieved by EA therapy. For instance, the consumption rate of sucrose solution was higher, the total distance and central distance in the open field test were increased, and EA treatment could reduce the immobility time of these rats. Swimming, struggling and immobility latencies were also increased in the forced swimming test. After EA treatment, the expression of 5-HT in the cerebral cortex and ileum increased. Finally, we found that EA ameliorated ischemia, depression, and gastrointestinal dysfunction in ISO-induced MH rats, which may be related to 5-HT metabolism.

Objective: Electroacupuncture (EA) pretreatment appears useful in the treatment of chronic myocardial ischemia (CMI). The goal of this study was to investigate the effect of EA preconditioning on the regulation of hypoxia-inducible factor (HIF)-1α and vascular endothelial growth factor (VEGF) proteins in a CMI model of vascular regeneration. Methods: A CMI model was established by subcutaneous injection of isoprinosine hydrochloride (ISO) for 14 days in 45 Wistar rats, which had been randomly divided into a model group ( n = 15), a CMI group pretreated with sham EA for 21 days (CMI + Sham group, n = 15) and a CMI group pretreated with verum EA for 21 days (CMI + EA, n = 15) prior to modeling. An additional 15 Wistar rats received 0.9% sodium chloride via intraperitoneal injection for 14 consecutive days (control group). Serum levels of VEGF and HIF-1α were measured by ELISA, while protein expression of VEGF and HIF-1α in the area of myocardial infarction was measured by Western blotting. The area of myocardial infarction and fibrosis of the myocardial tissue in the study groups were visualized by hematoxylin-eosin (HE) staining and Masson staining, respectively. Results: EA pretreatment improved cardiac function by regulating left ventricular end-diastolic diameter and left ventricular end-systolic diameter, left ventricular ejection fraction and the ST segment voltage of the electrocardiogram. EA pretreatment promoted vascular regeneration by increasing serum levels of VEGF and HIF-1α and by increasing protein expression of HIF-1α and VEGF in the infarcted region of the myocardium, leading to a reduction in the area of myocardial infarction on HE staining and reduction of myocardial fibrosis on Masson staining. Conclusion: EA pretreatment promotes protein expression of HIF-1α and VEGF in areas of ischemic myocardium, which may represent useful biomarkers for coronary collateral establishment and offer potential targets for therapeutic angiogenesis in patients with CMI.
