White Grain Mycetoma Caused by a Cylindrocarpon sp. in India

Abstract: We describe a case of white grain eumycetoma of the foot of an Indian male caused by a slow-growing, poorly sporulating fungus that does not match any known agent of this infection. Histologic examination of a biopsy tissue specimen showed oval, lobular, white granules composed of hyaline, septate hyphae, and thick-walled chlamydospores. Culture of granules from a draining sinus yielded compact, very-slow-growing, poorly sporulating colonies producing a strong reddish brown pigment that diffused into the mediu… Show more

“…Fusarium species may cause intoxication by invading grain. Besides, it may lead to infections like conjunctivitis, ceratitis, endophthalmitis, maxillary sinus infection, osteomyelitis, septic arthritis, brain abscess, colonization in burns and necrotic wounds, ulceration on legs, deep tissue infections, contact dermatitis and onychomycosis, though they are rare in immunsupressed patients [6][7][8][9].…”

“…When the etiology is fungal they are known as Eumycetomas. The organisms are traumatically implanted into the deep dermis or subcutaneous tissue from the natural environment and cause a subcutaneous infection characterized by the formation of large, macroscopic aggregates of fungal or actinomycetes filaments known as grains [1,2]. A dense infiltrate of Polymorphonuclear leukocytes and other inflammatory cells accumulates around these structures.…”

“…It does not spread beyond the locality of the initial site of infection and is seldom fatal. As there are pronounced differences in the treatment and prognosis of mycetomas resulting from different organisms, in particular between the eumycetomas and actinomycetomas, identification of causative agent plays a key part in their management [2][3][4]. Mycetoma is endemic in Africa, from Sudan to Somalia through Mauritania and Senegal.…”
