The aim of this letter is to share practical anatomical insights with surgeons who perform MUS, where possible, within the scope of the review. 1 More comprehensive practical insights can be accessed in a series of three consecutive related presentations in this journal "The mechanics of urethral closure, incontinence, and midurethral sling repair." | PATHOGENESIS OF SUIThe causation of SUI is purely mechanical. 5 With reference to Figure 1, weak pubourethral ligaments (PULs) cannot support the urethra and are stretched to "L during coughing by the action of the posterior muscle forces, levator plate (LP)/conjoint longitudinal muscle of the anus (LMA), which pull down the proximal vagina, bladder base and trigone to open the urethra. Video S1 describes the urethral closure mechanisms. Video S2 describes the pathogenesis of SUI.
