Amaç: İdyopatik orbita inflamasyonu sendromu (İOİS) ön tanısı ile oküloplasti polikliniğine yönlendirilen olgularda biyopsi sonuçlarımızı sunmak. Gereç ve Yöntem: 2009-2013 yılları arasında İOİS ön tanısı ile oküloplasti polikliniğine yönlendiren ve takibe giren 22 olgu irdelendi. Ayırıcı tanılar ekarte edilemeyen, steroid tedavisine yeterli yanıt vermeyen veya tedavi sonrası nüks görülen olgulara biyopsi yapıldı. Histopatolojik değerlendirme sonuçlarına göre tedavi protokolleri belirlendi. Bulgular: Olguların 12'si erkek 10'u kadındı. Ortalama yaş 45,6 (16-77 yaş aralığı) yıl idi. Sekiz olguda steroid tedavisine yetersiz yanıt veya nüks, dört olguda diğer ayırıcı tanılar ekarte edilememesi nedeni ile toplam 12 olguya biyopsi yapılması önerildi. Bir olgu işlemi kabul etmedi. Bir olgu eşlik eden sağlık problemi nedeni ile opere olamadı. On olguya biyopsi yapıldı. Lezyonların yerleşim özelliklerine göre yedi olguda orbital yağ ve diğer içerikten, iki olguda lakrimal bezden, bir olguda ekstraoküler kastan biyopsi alındı. Sekiz olgu histopatolojik değerlendirme sonucu idyopatik orbital inflamasyon tanısı ile uyumlu iken, bir olguda retrobulber alanda, diğer olguda da lakrimal bezden alınan örnekler lenfoma olarak rapor edildi. Sonuç: İOİS ön tanısı ile başvuran olgularda, diğer ayırıcı tanılar ekarte edilemiyorsa, steroid tedavisine yeterli yanıt yoksa veya tedavi sonrası nüks görülüyorsa, olgulara biyopsi yapılmasının, tanının kesinleştirilmesinde ve tedavi planlamasında gerekli olduğu görülmüştür. Materials and Methods: We evaluated 22 patients who were referred to our oculoplastic clinic with an initial diagnosis of idiopathic orbital inflammation between 2009 and 2013. We performed biopsy on the patients in whom a differential diagnosis was not possible, or response to steroid treatment was not satisfactory, or recurrence occurred after steroid treatment. According to the histopathological evaluation, treatment protocols were determined. Results: The study included 12 male and 10 female patients. Mean age was 45.6 years (range 16-77 years). Inadequate response to steroid treatment or recurrence in 8 patients and difficulty in differential diagnosis in 4 patients were the indications for the biopsy. One patient declined the biopsy. One patient could not be operated on because of the accompanying systemic diseases. We performed orbital fat tissue biopsy in 7 patients, lacrimal gland biopsy in 2 patients, and extraocular muscle biopsy in 1 patient. Histopathological evaluations demonstrated IOIS in 8 patients. Lymphoma was diagnosed in 2 patients in whom biopsies were taken from the lacrimal gland and retrobulbar fat tissue. e report the clinical course and pathologic findings in a case of intraocular sclerosing inflammatory pseudotumor in a 21-year-old man. The patient initially had a unilateral right interstitial keratitis, scleritis, uveitis, ciliary body mass, and retinal detachment. Scleral and vitreous biopsy specimens revealed an inflammatory process. The eye was eventually enucleated despite therapy with high doses of prednisone and ciprofloxacin hydrochloride. Histologic examination of the globe showed nongranulomatous, acute (neutrophils) and chronic (lymphocytes and histiocytes) inflammation with proliferation of fibrous tissue within the vitreous cavity, uvea, sclera, and contiguous orbital fibroadipose tissue. The contralateral eye later developed a similar mass that resolved following aggressive and prolonged immunosuppressive therapy with retention of 20/16 visual acuity.
