What determines length of stay after total hip and knee arthroplasty? A nationwide study in Denmark

Abstract: Nationwide implementation of fast-track THA and TKA would result in a significant decrease in the needed number of hospital beds with similar or better outcome for the patients. Implementation of updated logistical and clinical features is expected to increase rehabilitation and reduce LOS with similar or improved patient satisfaction. These results support the implementation of fast-track total hip- and knee arthroplasty.

“…Finally, the very high response rate also strengthened the study. A limitation of the study was that the questionnaire was only pilot-tested by the Danish Health and Medicine Authority (Danish Health and Medicine Authority 2006, Husted et al 2010a) and was not validated using an accepted validation method. The questionnaire could be limited in terms of a ceiling effect (Taylor 2010), because a high proportion of the patients gave the highest possible score on some of the questions.…”

“…The questionnaire (Table 1) was developed and pilot-tested by the Danish Health and Medicine Authority (Danish Health and Medicine Authority 2006, Husted et al 2010a). Satisfaction outcomes were measured on a numerical rating scale (NRS) from 0 (not satisfied at all) to10 (best possible satisfaction).…”

“…The length of hospital stay for patients undergoing hip replacement has decreased (Gulotta et al, 2011, Husted et al, 2010 which has put greater demands on perioperative practice, including patient education. This can be a challenge for the patients and it is therefore important to take their own resources, e.g.…”

“…Some of the causes of increased postoperative LOS are modifiable while others are not. Previous studies of risk factors associated with prolonged LOS after arthroplasty describe female gender, increasing age, comorbidities, obesity, higher American Society of Anesthesiologists-Physical Status scores, longer incisions, longer operative time, the need for transfusion, and postoperative complications as being associated with increased LOS [2,3,5,7,[10][11][12].…”
