Background Large-scale emergencies hurt the chronically ill immensely. Improving personal emergency preparedness is an effective means to protect themself. Our objective is to evaluate the emergency preparedness level and its influencing factors among patients with chronic diseases in China. Methods A paper-based self-administrated questionnaire collected data. Descriptive statistics and univariate analysis were used to illustrate the chronically ill's general characteristics and preparedness level. Regression models were constructed to identify influencing factors of preparedness. Results A total of 771 samples were included. Only 23% of respondents prepared three days of water, three days of food, available flashlights, and available radios. 50% were equipped with three-day prescription drugs. Patients with chronic respiratory diseases or cancer were less prepared (p < 0.01). Patients who were never married or married (compared with widowed, divorced, or separated); had junior high school degrees (compared with a college degree and above); had monthly household incomes of more than 10,000 yuan; suffered from two or above chronic diseases, often and rarely discussed emergencies with families (compared with never); perceived learning emergency-related information as essential; participated in emergency drills; believed their families have prepared; and had high levels of mental health are more likely to have the four preparations. Factors related to higher levels of prescription drug reserve include owning junior high school or high school degrees, having two or above chronic diseases, being sick for five to nine years (compared to less than or equal to one year), being interested in gathering information about the emergency response, and perceived learning emergency-related information is crucial. Conclusion Patients with chronic diseases have a low level of emergency preparedness. Public health officials should formulate tailored policies, information, and intervention measures that target chronically ill patients to improve their emergency preparedness, particularly those with low income, low mental health, divorce, widowhood, or separation.

Prehospital major incident management: how do training and real-life situations relate? A qualitative study

ObjectiveTo explore the relationship between preparations and real-life experiences among prehospital major incident commanders.DesignAn explorative, qualitative design was used.SettingPrehospital major incidents in Sweden. Data were collected between December 2019 and August 2020.ParticipantsPrehospital major incident commanders (n=15) with real-life experiences from major events, such as fires, bus accidents, a bridge collapse and terrorist attacks, were included. All but one had participated in 2-day training focusing on the prehospital management of major incidents. In addition, about half of the participants had participated in simulation exercises, academic courses and other training in the management of major incidents.MethodsData from two-session individual interviews were analysed using inductive thematic analysis.ResultsThe conformity between real-life major incidents and preparations was good regarding prehospital major incident commanders’ knowledge of the operational procedures applied in major incidents. However, the preparations did not allow for the complexities and endurance strategies required in real-life incidents. Personal preparations, such as mental preparedness or stress management, were not sufficiently covered in the preparations. To some extent, professional experience (such as training) could compensate for the lack of formal preparations.ConclusionsThis study identified perceived gaps between preparations and real-life experiences of being a prehospital major incident commander. To minimise the gaps between demands and expectations on perceived control and to better prepare individuals for being prehospital major incident commanders, the training and other preparations should reflect complexities of real-life incidents. Preparations should develop both technical skills required, such as principles and methodology used, and personal preparedness. Personal preparations should include improving one’s mental preparedness, self-knowledge and professional self-confidence required to successfully act as a prehospital incident commander. Since little is known about what pedagogical methods that should be used to enhance this, further research is needed.

Objective: Emergency preparedness and response operations for all types of catastrophes rely heavily on healthcare facilities and their staff. On the other hand, hospital employees suffer significant gaps in emergency preparedness knowledge and skills when it comes to treating mass casualties. The objective of this study was to assess the nurses’ and physicians’ familiarity with emergency preparedness and identify the associated factors. Methods: A facility-based cross-sectional survey was conducted by census utilizing a self-administered questionnaire among all nurses and physicians working in emergency departments in East Gojjam zone public hospitals. The collected data were entered into Epi-data version 4.2 and exported to SPSS 25.0 for further analysis. Frequency, mean, and standard deviation were computed to describe individual and other characteristics of the sample. A simple and multiple linear regression model was fitted to identify factors associated with familiarity with emergency preparedness. An unstandardized adjusted beta ( β) coefficient with a 95 % confidence level was used to report the result of the association at a p-value of 0.05 statistical significance. Results: In this study, a total of 237 individuals completed the questionnaire, yielding a response rate of 94 %. The mean score of familiarity with emergency preparedness was 106.1 ± 31.8 (95% CI: 102, 110.1), with approximately 52.3 % scoring higher than the mean score. Self-regulation (B = 3.8, 95% CI: 2.6, 5), health care climate (B = 1.4, 95% CI: 0.4, 2.43) and participation in actual major disaster event (B = 15.5, 95% CI: 7.8, 23.2) were significant predictors of familiarity. Conclusion: According to the findings of this study, nurses’ and physicians’ expertise in emergency and disaster preparedness is inadequate. Previous engagement in actual disaster events, self-regulation, and the healthcare climate were significant predictors of familiarity. As a result, the responsible stakeholders should develop strategy to enhance self-regulation (motivation), job satisfaction of emergency department employees, and drills and hands-on training in mass casualty management.
