Vulvar involvement in pediatric Crohn’s disease: a systematic review

Abstract: This review shows that pediatric VCD is an uncommon disease, difficult to be diagnosed as either symptoms or clinical lesions are not specific. A multidisciplinary approach is advised to reach a correct diagnosis and plan clinical treatment.

“…Vulvoperineal manifestations of CD are rare and clinically present as fistulae, ulcerations, abscesses, perianal skin tags, fissures, and diffuse edema 7 . These findings are more commonly seen in pediatric patients than in adult patients 8 . Such manifestations may present clinically as an isolated EIM of CD 9 or as the initial presentation of CD 10 .…”

“…3 An early vulvar involvement of CD in children is extremely rare and our patient is among the few cases reported in the paediatric population. 4 Although most children with vulvar CD are asymptomatic, our patient's constant genital discomfort was triggered by the progressive worsening of the vulvar swelling. This sign may be caused by granulomatous inflammation within the vulva or granulomas blocking the lymph vessels, leading to lymphoedema.…”

“…2 Symptoms of metastatic CD are rare and can actually precede the classic symptoms of CD. 3,4 When this occurs, pediatricians and sub-specialists are often faced with diagnostic dilemmas and patients consequently suffer from prolonged symptoms and extraneous treatments. 5 In this article, we report the case of a pediatric patient who presented to dermatology for vulvar swelling and was ultimately diagnosed with metastatic CD.…”
