Vulval squamous cell carcinoma arising in chronic hidradenitis suppurativa

Abstract: We report a case of vulval squamous cell carcinoma (SCC) arising in chronic hidradenitis suppurativa (HS). The patient had a complex medical history including a 25-year-history of Crohn's disease. In addition she had recently received immunosuppressive therapy for nephrotic syndrome secondary to membranous glomerulonephritis. A painful nodule was noted on the vulva that was clinically very suspicious of SCC. An excision biopsy confirmed the diagnosis. There are few publications in the English literature citing… Show more

“…Other investigators have highlighted the fact that this inflammatory lesion can occur in patients with long-standing Crohn's disease, and have reported a case of chronic HS complicated by a vulval squamous cell carcinoma [181]. Finally, Maclean et al, referring to advanced cases of squamous cell carcinoma (SCC) arising in chronic HS, have advocated that HS arising in extra-axillary sites is a pre-malignant condition [182].…”

“…In addition to scarring, SCC has been reported in longstanding disease (Short et al 2005), and one case of squamous cell carcinoma with hidradenitis was associated with a paraneoplastic neuropathy (Rosenzweig et al 2005). Verrucous carcinomas have also been described (Cosman et al 2000), and one case of clear cell acanthoma (Zedek et al 2007).Other complications in severe disease include anaemia, arthropathy, urethral fistula and amyloidosis (Montes-Romero et al 2008).…”

“…Although distant metastasis is generally rare in SCC secondary to HS, metastatic sites reported in the literature include the axial skeleton, abdominopelvic viscera, peritoneum, vulva, and lung parenchyma [1,4,8]. In our patient, the metastatic disease was to the pleura.…”
