Volumetric modulated arc therapy (VMAT) for skin field cancerisation of the nose - A technique and case report

Abstract: Locally advanced skin cancer of the nose is problematic, especially when all nasal skin needs treatment as in the case of skin field cancerisation (SFC). Surgery is considered the gold standard but involves tissue loss which may impact function and cosmesis. In patients who are not good surgical candidates, definitive radiotherapy (RT) can be used.Traditional RT modalities for the nose, such as brachytherapy, superficial x-ray treatment or electrons, have quality assurance issues and can be difficult to confor… Show more

“…The patient tolerated the prescribed treatment of 55 Gy in 25 fractions daily (Monday-Friday) over five weeks using 6 MV as per our published technique 1 and case study. 14 Figure 5 shows the extent of the RT field on the left side of nose at simulation. In vivo dosimetry showed the planned dose was delivered.…”

“…Higher dose delivery is possible using a simultaneous integrated boost (SIB) approach. 11 Draining lymph node fields need palpating, and signs of perineural invasion need to be excluded. Investigations will be driven by the history and examination.…”

“…The areas drawn on the patient are copied onto plastic sheets as templates 11 which are then stored electronically in the patient's file. This record allows fields to be reproduced in the future to determine if disease recurrence is in-field, at field edge, or out of field.…”

“…9 New radiation techniques have been developed, especially volumetric modulated arc therapy (VMAT) which is a further evolution of intensity modulated radiotherapy (IMRT). [10][11][12] VMAT allows definitive radiation treatment of large convex fields (e.g. scalp, forehead, cheeks, forearms and legs).…”
