A radiation oncology approach for using definitive radiotherapy with volumetric modulated arc therapy (VMAT) for skin field cancerisation (SFC)

Abstract: Patients with long histories of chronic ultraviolet radiation exposure can suffer from skin field cancerisation (SFC). They can have poor oncological outcomes and quality of life (QoL) with itch, flaking skin, and suboptimal cosmesis. Current treatments have high rates of treatment failure and non-compliance. Definitive radiotherapy (RT) with volumetric modulated arc therapy (VMAT) has been proposed as a possible treatment modality in this clinical setting. This article provides a theoretical background to ass… Show more

“…6 Standard fractionation of 2 gray (Gy) per day or less means that the acute radiation side effects occurring at any given time in treatment corresponds to the dose delivered at that stage. 7 Hypofractionated courses, common in skin cancer, often have a component of acceleration and toxicity can peak after the dose is given. This means titration to the acute reaction of the day cannot be done.…”

“…Skin patients are often different from tradition RT patients. 35 They have chronic conditions that cause more morbidity than mortality. They can have comorbidities that promote skin cancer (e.g.…”

“…9 New radiation techniques have been developed, especially volumetric modulated arc therapy (VMAT) which is a further evolution of intensity modulated radiotherapy (IMRT). [10][11][12] VMAT allows definitive radiation treatment of large convex fields (e.g. scalp, forehead, cheeks, forearms and legs).…”
