Vitreous Substitutes: Old and New Materials in Vitreoretinal Surgery

Abstract: Recent developments in vitreoretinal surgery have increased the need for suitable vitreous substitutes. A successful substitute should maintain all the physical and biochemical properties of the original vitreous, be easy to manipulate, and be long lasting. Substitutes can be gaseous or liquid, both of which have associated advantages and disadvantages related to their physical properties and use. Furthermore, new surgical techniques with smaller vitreoretinal instruments have driven the use of more viscous su… Show more

“…More details about these vitreous substitutes can be found in several review articles. 53,[144][145][146][147][148][149][150][151][152][153][154] Most current experimental hydrogel vitreous substitutes have moduli that are orders of magnitude higher than those of the human vitreous (Table 4 and Figure 4A). However, stiff hydrogels can be difficult to inject through a small gauge needle into the vitreous chamber.…”

“…And Vitreous tamponade aims to replace the natural vitreous body with vitreous substitutes to support the retina, promote the anatomical reduction of the retina and prevent the eyeball from shrinking [5,12]. But current discovered vitreous substitutes have short-term life-span and high toxicity causing severe complications in the eyes [13,14]. Specially, SO has been proved to be toxic for ciliary body and attribute to emulsificate in the eyeball, though SO has the most stable physical and chemical attributions among all the vitreous substitutes [15].…”

“…Cross-linked hydrogels are synthetic polymer networks that are expanded throughout their volume by water. Smart hydrogels can create a three-dimensional structure characterized by transparency, biocompatibility, and mechanical flexibility [ 196 ]. These polymers have only been used on an experimental basis and, at this time, their major disadvantage is the intravitreal inflammation due to immune system activation [ 197 ].…”
