Objectives: Little is known about the association of vitamin D and alopecia areata (AA). Our objectives were to search a relation between 25-hydroxyvitamin D [25(OH)D] levels and the development of AA and the efficacy of oral vitamin D treatment in children with AA and vitamin D deficiency.Methods: Thirty newly diagnosed AA patients and 30 sex-and age-matched controls were included in the study. Levels of 25(OH)D, parathormone, calcium, inorganic phosphate, alkaline phosphatase were measured at baseline and sixth month. Both patients and controls who diagnosed vitamin D deficiency were treated with oral vitamin D for six months.Results: Serum 25(OH) D levels of the patients and controls were 25.3 ± 19.4 ng/ml and 21.3 ± 12.5 ng/ml, respectively (p>0.05). The frequency of vitamin D deficiency was similar in patients and controls. Serum levels of 25(OH)D and calcium were increased significantly after six months of the treatment in both patients and controls with vitamin D deficiency (p<0.05). A higher frequency (47%) of complete improvement was observed in patients with AA and vitamin D deficiency during oral vitamin D treatment. Conclusions:There was no statistically significant difference in 25(OH)D levels between the patients with AA and controls. However, we observed a higher frequency of complete improvement in these patients with an improved vitamin D status. Thus, oral vitamin D treatment can be given only to selected AA patients who are also deficient in vitamin D.

Przegląd Dermatologiczny 2016/2 StreSzczenieŁysienie plackowate jest chorobą zapalną charakteryzującą się niebliznowaciejącą utratą włosów, której patogeneza, pomimo częstego występowania u pacjentów dermatologicznych, nie została dostatecznie poznana. Ostatnio podkreśla się udział procesów autoimmunologicznych, na co wskazuje obecność autoprzeciwciał skierowanych przeciw antygenom mieszków włosowych, a także nacieki zapalne ze wzmożo-ną ekspresją cytokin w otoczeniu mieszka. Witamina D spełnia wiele ważnych funkcji w organizmie człowieka -odpowiada za utrzymanie prawidłowej homeostazy wapnia, jest także ważnym regulatorem odpowiedzi immunologicznej. Jej działanie odbywa się głównie za pośrednictwem jądrowego receptora zlokalizowanego na powierzchni komórek dendrytycznych, makrofagów, limfocytów B, limfocytów T, keratynocytów oraz na komórkach brodawki mieszka włosowego. Witamina D działa supresyjnie na komórki układu immunologicznego, a jej niedobór może mieć znaczenie w procesach patofizjologicznych łysienia plackowatego. Dotychczasowe dane sugerują, że suplementacja witaminą D 3 jest wartą rozważenia opcją terapeutyczną u chorych na łysienie plackowate. AbStrActAlopecia areata is an inflammatory disease with noncicatricial hair loss. Despite its high prevalence in the dermatological patient population, the pathogenesis is not sufficiently understood. In recent years, the contribution of autoimmune processes has been emphasized, as indicated by the presence of autoantibodies against hair follicle antigens and inflammatory cell infiltrates with increased expression of cytokines around the hair follicle. Vitamin D performs many important functions in the human body -is responsible for maintaining calcium homeostasis but is also an important regulator of the immune response. It acts mainly via the nuclear vitamin D receptor located on the surface of dendritic cells, macrophages, B and T cells, keratinocytes and cells of the hair follicle papillae. Vitamin D suppresses the immune system cells, and its deficiency may be important in pathophysiological phenomena in alopecia areata. According to current data, vitamin D supplementation may be a therapeutic option worth considering in patients with alopecia areata.
