Visualizing Macular Structures During Membrane Peeling Surgery With an Intraoperative Spectral-Domain Optical Coherence Tomography Device

Abstract: mi-SD-OCT appears to be a valuable tool for intraoperative visualization of the ERM and offers immediate visualization of retinal anatomy during peeling. Therefore, it adds to the understanding of intraoperative traumatic changes due to the peeling procedure.

“…In comparison to these findings, we previously reported about tent‐shaped intraoperative subfoveal hyporeflective zones (Leisser et al. ) corresponding to traction during peeling and typically disappearing directly after ERM is removed. To our knowledge, there are no previous reports about outcomes of patients having had intraoperative subfoveal hyporeflective zones during macular surgery.…”

“…In 56 patients, image quality was sufficient for evaluating whether an intraoperative subfoveal hyporeflective zone, a pathology induced by traction during peeling (Leisser et al. ), was present. Usually, intraoperative subfoveal hyporeflective zones disappear directly after resolving traction on the retina when ERM are removed.…”

“…iOCT guidance helped in finding starting points for the peeling and in visualizing areas with ERM leftovers [30-33]. Furthermore, iOCT guidance during peeling gave insights into the changes of retinal morphology during peeling, for example due to traction on the retina [31]. …”

“…Nevertheless, improvement of visual acuity was comparable between groups. Due to the fact that the ILM usually cannot be visualized using iOCT imaging [31], chromovitrectomy still plays an important role in macular surgery, at least in cases where ILM peeling is mandatory. To further examine the effects of subfoveal hyporeflective zones on postoperative outcomes, studies comparing pre- and postoperative microperimetry results will have to be performed.…”
