Viral exanthem with “Spins and needles sensation” on extremities of a COVID-19 patient: A self-reported case from an Indonesian medical frontliner

Abstract: Cutaneous manifestation is a newly reported clinical manifestation of COVID-19 infection. The clinical description of cutaneous manifestation is still not fully described. Our patient, a medical person, had viral exanthem distributed in the extremities along with a "Spins and needles sensation," which differs from a previously published paper on cutaneous manifestations. “…In one patient, oral lesions appeared simultaneously with systemic symptoms and in other patients, latency time was between 2 and 10 days. One patient had positive history of recurrent aphthous stomatitis (RAS) and two patients had positive PCR for herpes simplex virus (HSV) 4‐8,37 . Aphthous‐like lesions without necrosis were observed in younger patients with mild infection, whilst aphthous‐like lesions with necrosis and hemorrhagic crusts were observed more frequently in older patients with immunosuppression and severe infection.…”

“…Interestingly, there is one self-reported case of a doctor with COVID-19 complaining of 'pin and needles' sensation in palms and soles with subsequent appearance of acral macules similar to our study patients. 10 In COVID-19 patients, palms and soles provide useful information and should be routinely checked. Redness, swelling of the hands and feet along with burning sensation (erythrodysesthesia), fine desquamation and acral macules may support the diagnosis of COVID-19.…”
