The intrarectal suture is considered a high technically complex procedure. The study's objectives were to assess the feasibility of making an intrarectal knot, through an in vitro study and assessing whether the video tutorial facilitates learning. Materials and Methods:A detailed description of the technique. A comparative observational cross-sectional study in surgeons with no previous experience in intrarectal knots.Results: Twenty-one of these 32 participants passed the intrarectal knot test without video tutorial (T1) (65.6%), and 26 (81.2%) after the video tutorial (T2) (P = 0.26). The mean time taken to tie the knot fell from 74 seconds (SD = 46) in T1 to 41 seconds (SD = 41) in T2 (P < 0.001). At T1, 26 participants (81.3%) described the technique as difficult, but only 7 (21.9%) at T2 (P < 0.001).Conclusions: Performing the intrarectal knot suture is feasible. Despite the technical difficulty, the video tutorial is sufficient for surgeons to learn the technique.

There is a lack of training curricula and educational concepts for robotic-assisted surgery (RAS). It remains unclear how surgical residents can be trained in this new technology and how robotics can be integrated into surgical residency training. The conception of a training curriculum for RAS addressing surgical residents resulted in a three-step training curriculum including multimodal learning contents: basics and simulation training of RAS (step 1), laboratory training on the institutional robotic system (step 2) and structured on-patient training in the operating room (step 3). For all three steps, learning content and video tutorials are provided via cloud-based access to allow self-contained training of the trainees. A prospective multicentric validation study was conducted including seven surgical residents. Transferability of acquired skills to a RAS procedure were analyzed using the GEARS score. All participants successfully completed RoSTraC within 1 year. Transferability of acquired RAS skills could be demonstrated using a RAS gastroenterostomy on a synthetic biological organ model. GEARS scores concerning this procedure improved significantly after completion of RoSTraC (17.1 (±5.8) vs. 23.1 (±4.9), p < 0.001). In step 3 of RoSTraC, all participants performed a median of 12 (range 5–21) RAS procedures on the console in the operation room. RoSTraC provides a highly standardized and comprehensive training curriculum for RAS for surgical residents. We could demonstrate that participating surgical residents acquired fundamental and advanced RAS skills. Finally, we could confirm that all surgical residents were successfully and safely embedded into the local RAS team.

Zusammenfassung Einführung Der Einsatz von Robotern in der minimalinvasiven Chirurgie hat in den letzten Jahren immer weiter zugenommen. Die roboterunterstützte Pankreaschirurgie hat gegenüber der laparoskopischen Pankreaschirurgie den Vorteil einer besseren Handhabung und verbesserten Angulation durch die größere Flexibilität der Instrumente. Darüber hinaus besteht durch die 3-D-Technik eine verbesserte Sicht und durch eine softwarebasierte Tremorkontrolle eine exakte Instrumentenführung. Die körperliche Belastung des Chirurgen durch die sitzende Position ist bei langen Eingriffen geringer. Methoden und Ergebnisse Dieser Artikel liefert einen Ansatz für die Einrichtung eines Roboter-Pankreas-Programms und eine detaillierte Anleitung im Sinne von SOPs für die robotergestützte Pankreaslinksresektion und robotergestützte Pankreatoduodenektomie. Ergebnisse In unserem standardisierten SOP-Ansatz verwenden wir sowohl bei der Pankreaslinksresektion als auch bei der Pankreatduodenektomie jeweils 5 Trokare – 4 Da-Vinci-Trokare und einen laparoskopischen Hilfstrokar. Wir bevorzugen die Position der Robotertrokare auf einer geraden horizontalen Linie mit einem Abstand von etwa 20 cm vom Operationsfeld. Die Operation wird in standardisierten Arbeitsschritten durchgeführt. Bei der Pankreaslinksresektion sind dies: Zugang und Visualisierung des Pankreas, Mobilisierung der linken Dickdarmflexur und des Pankreasunterrandes, Präparation der V. lienalis und der mesenterikoportalen Achse, Präparation des Pankreasoberrandes mit Lymphadenektomie und Darstellung der A. lienalis, Durchtrennung des Pankreas, retrograde Präparation des Pankreas und der Milz (bei onkologischen Eingriffen). Bergung, Einlage von Drainagen und Wundverschluss. Bei der Pankreatoduodenektomie sind es folgende Schritte: Zugang und Visualisierung des Pankreas, erweitertes Kocher-Manöver, Darstellung der Unterkante und der mesenterikoportalen Achse, Darstellung des Pankreasoberrandes und des Lig. hepatoduodenale, Präparation des Pankreashalses, Darstellung der Mesenterialwurzel und Pars IV duodeni, Präparation des Mesopankreas, Durchführung der Pankreasanastomose und der biliodigestiven Anastomose, Naht der dudenojejunalen Anastomose, Einlage von Drainagen und schichtweiser Wundverschluss. Der graduelle Aufbau unseres Roboter-Pankreas-Programms und der strukturierte Ansatz für komplexe Pankreasresektionen werden erläutert. Zusammenfassung Zusammenfassend beschreiben wir den Ansatz der robotischen Bauchspeicheldrüsenchirurgie in einem minimalinvasiven Exzellenzzentrum und Bauchspeicheldrüsenzentrum auf struktureller und verfahrenstechnischer Ebene, um die Einrichtung derartiger Programme an anderen Standorten zu unterstützen.
