Very rare case of synchronous volvulus of the sigmoid colon and caecum causing large-bowel obstruction

Abstract: Large-bowel obstruction caused by volvulus is potentially life threatening if not managed promptly. Sigmoid colon is the most common site of volvulus followed by caecum. Synchronous double colonic volvulus is an exceedingly rare entity, with only five cases published previously. We present the case of a 77-year-old man with a synchronous sigmoid and caecal volvulus. He underwent total colectomy with ileostomy formation with a view to reverse the ileostomy at a later date.

“…In all these types, there is potential for intestinal obstruction and strangulation [11,24] with the risk higher in types 1 and 2 which constitute about 80% of caecal volvulus. Caecal volvulus may sometimes coexist with a synchronous splenic flexure volvulus [25] and/or sigmoid volvulus [26].…”

“…In patients with poor physiological reserve, a conservative approach such as limited ileocaecal resection or formation of an ileostomy may reduce intraoperative time and result in better postoperative outcome [47]. In synchronous caecal and sigmoid volvulus, a total colectomy with ileostomy may sometimes be necessary [26].…”

“…Roy et al . [ 3 ] also attempted conservative management in a stable patient with flexible sigmoidoscopy and rectal tube insertion. This was ultimately unsuccessful with recurrence following removal of the rectal tube.…”

“…The decision for ileostomy was based on the presence of multiple risk factors for anastomotic leak in these patients, including age, gender, comorbidities and intraoperative findings [ 4 ]. Definitive surgical management described in previous case reports was usually subtotal colectomy with either end ileostomy [ 3 , 5 ] or ileorectal anastomosis [ 6 , 7 ]. Singh et al .…”

“…There are approximately 100 cases of TCV reported in the medical literature with many of them discovered at the time of surgery [2] . Synchronous double volvulus of the sigmoid colon and cecum has been reported in the literature [3 , 4] , but never a synchronous double volvulus of the transverse colon and cecum. We present the case of a 26-year-old woman with a history of scleroderma-systemic erythematosus lupus (SLE) overlap syndrome who was discovered to have synchronous volvulus of the transverse colon and cecum on CT imaging.…”
