Ventricular Dysfunction and the Risk of Stroke after Myocardial Infarction

Abstract: During the five years after myocardial infarction, patients have a substantial risk of stroke. A decreased ejection fraction and older age are both independent predictors of an increased risk of stroke. Anticoagulant therapy appears to have a protective effect against stroke after myocardial infarction.

“…This value seems to be lower than that described for ischemic cardiopathy, where the incidence of CE ischemic stroke was 1.1 to 1.8% a year [7][8][9][10][11] . However, these studies included only higher-risk patients, with moderate to severe systolic dysfunction.…”

“…Within the model of cardioembolic stroke, the only subgroup with a well-established proposal of prophylaxis is the atrial fibrillation one 6 . Another model that has been described, the dilated cardiomyopathy model [7][8][9][10][11] , has not presented prospective studies capable of defining prevention strategies. In our country, the chronic chagasic cardiopathy can significantly contribute to the genesis of this lethal and certainly incapacitating event, maybe due to its highly emboligenic nature, justified by the innate characteristics of the cardiopathy [12][13][14][15] .…”

“…Coronary artery disease patients with left ventricular hypertrophy have 3 times the risk of stroke, whereas coronary artery disease patients with congestive heart failure have 4 times the risk [19]. Within 5 years of a myocardial infarction, the rate of stroke is 8.1% [28], and older patients or patients with a cardiac ejection fraction less than 28% are at higher risk of stroke.…”

“…[35] In the SAVE and SCD-HeFT studies higher rates of stroke and thromboembolic event rates correlated with greater degrees of left ventricular systolic dysfunction. [36], [37] Among patients with a borderline CHA 2 DS 2 -VASc score, assessing for the presence or absence of additional thromboembolic risk factors might prove advantageous.…”
