Vasopressin improves survival in a pig model of hypothermic cardiopulmonary resuscitation

Abstract: A single 0.4-unit/kg dose of vasopressin administered at a body core temperature <30 degrees C significantly improved defibrillation success and 1-hr survival in a pig model of hypothermic cardiopulmonary resuscitation.

“…44 The guidelines of the European Resuscitation Council recommend a modified approach to advanced life support, consisting of up to three defibrillations, with epinephrine withheld until the core temperature is higher than 30°C (86°F) and with the interval between doses doubled until the core temperature is higher than 35°C (95°F). 8 These recommendations conflict with the American Heart Association guidelines, which state, "It may be reasonable to consider administration of a vasopressor during cardiac arrest according to the standard ALS [advanced life support] algorithm concurrently with rewarming strategies."…”

“…Although the international CPR guidelines recommend no vasopressor drugs when body temperature is Ͻ30°C, we have shown in hypothermic animal models that vasopressors indeed make successful defibrillation more likely, especially when combined with rewarming (13). This has been shown in another case report of a near-drowning accident, when restoration of circulation was possible after administration of a combination of epinephrine and vasopressin at a core body temperature of 27.4°C (14).…”

Στην πρώτη ομάδα η στεφανιαία πίεση, η επανάκτηση αυτόματης κυκλοφορίας και η επιβίωση ήταν σημαντικά υψηλό τερες (Schwarz et al 2002).…”
