Vascular complications following liver transplantation: A literature review of advances in 2015

Abstract: Although vascular complications (VCs) following orthotopic liver transplantation (OLT) seldom occur, they are the most feared complications with a high incidence of both graft loss and mortality, as they compromise the blood flow of the transplant (either inflow or outflow). Diagnosis and therapeutic management of VCs constitute a major challenge in terms of increasing the success rate of liver transplantation. While surgical treatment used to be considered the first choice for management, advances in endovasc… Show more

“…In our study, the indications comprised arterial complications (6 patients of hepatic artery thrombosis and 2 patients of intima dissection), portal venous complications (3 patients of portal venous thrombosis, 3 patients of strictures, and 1 patient of shunt ligation due to a low portal flow), and 2 patients of venous outflow block due to rotation of the graft. Not surprisingly, the outcome of such patients was significantly worse than that of the nonrelaparotomy group . Recently, due to the advancement of interventional radiology, successful experiences of endovascular treatment for vascular complications have been reported .…”

“…Not surprisingly, the outcome of such patients was significantly worse than that of the nonrelaparotomy group. (16) Recently, due to the advancement of interventional radiology, successful experiences of endovascular treatment for vascular complications have been reported. (16,17) Because our experiences were limited, we did not compare the difference in outcome between relaparotomy and endovascular treatment.…”

“…In pediatric cases LDLT is the preferred modality. Problems specific to pediatric LDLT are large for size graft and higher incidence of Hepatic Artery Thrombosis (HAT) [4]. Various refinements in techniques have been described to avoid these complications.…”
