Various rheumatic syndromes in adult patients associated with high antistreptolysin O titres and their differential diagnosis with rheumatic fever.

Abstract: Objectives-The purpose of this study was to analyse retrospectively adult patients with acute joint or muscle symptoms and a high antistreptolysin 0 (ASO) titre to find out which syndromes of clinical arthritis are associated with serological evidence of streptococcal infection. Methods-Seventy six adult patients with an acute arthritis syndrome or an exacerbation in their chronic rheumatic disease and simultaneously a high ASO titre (¢500 Todd units) were

“…It is interesting to note that the results of this study are very similar to the findings of our earlier report on patients with both an acute arthritis syndrome and a high antistreptolysin O titre (30) suggesting that streptococcal and also staphylococcal infections can associate with acute phases of many kinds of rheumatic syndromes.…”

“…In a study by Valtonen et al. 5 reactive arthritis, rheumatoid arthritis, ankylosing spondylitis, as well as psoriatic arthritis, were all associated with high titers of serum ASO.…”

“…Arthritis following primary throat infection with ␤HSA applies to this definition. Recent papers suggest that a spectrum of reactive post-streptococcal diseases may become manifest secondary to ␤HSA infection [3][4][5][6][7][8][9]. Though arthritis may be very painful in post-streptococcal disease, the complication which is most feared in ARF is a devastating carditis.…”

“…At present, there is an ongoing discussion whether post-streptococcal reactive arthritis (PSRA) is a forme fruste of ARF or whether it is a separate sequel to ␤HSA infection with a very low or possibly even absent risk of carditis [9,[16][17][18][19]. Long-term antibiotic prophylaxis in ARF is currently advocated in order to reduce a progressively increasing risk of carditis in subsequent episodes of ARF [1].…”
