Variability of IgE reactivity profiles among European mite allergic patients

Abstract: Purified Der p 1 and Der p 2 are sufficient for the diagnosis of > or = 97% of D. pteronyssinus allergic patients in Europe, but other allergens may also play an important role for the diagnosis and treatment of HDM allergy.

“…Still, there is a considerable level of Ab cross-reactivity between the closely related group 2 HDM allergens Der p 2, Der f 2, and Eur m 2 (.80% amino acid sequence identity) (4,(8)(9)(10)(11). Conversely, Ab cross-reactivity between Der p 2 and the more distant related group 2 storage mite allergens Lep d 2, Tyr p 2, and Gly d 2 is low, consistent with a low (40%) sequence similarity between Der p 2 and these allergens (2,9).…”

“…The frequency of IgE recognition, allergenic activity and clinical relevance of the major HDM allergens, Der p 1 and Der p 2, has been well investigated [5][6][7][8]. Both allergens are recognized by more than 90% of HDM-allergic patients and are considered as the major HDM allergens [5]. More than 20 other mite allergens have been identified but their importance for mite allergy has only partly been studied [9].…”

“…HDM allergens are among the most common triggers of asthma and allergic rhinitis [3,4]. Both allergens are recognized by more than 90% of HDM-allergic patients and are considered as the major HDM allergens [5].…”

“…In fact, it is known that a mix of Der p 1 and Der p 2 allows diagnosing the majority of HDM allergic patients (30). However, a molecular or epitope mixture for specific immunotherapy certainly needs to include Der p 23 because of its high allergenic activity, its high frequency of recognition and due to the high levels of IgE-antibodies directed to this allergen.…”

“…The diagnosis of allergy to D. pteronyssinus was based on case history, typical indoor symptoms (rhinitis, conjunctivitis, and/or asthma), and the presence of D. pteronyssinusspecific IgE Abs > 0.7 kU A /l in serum as determined with the ImmunoCAP System (Thermofisher/Phadia, Uppsala, Sweden). Sera from nonallergic individuals were used for control purposes (30). The IgE binding frequency of Der p 23 was determined in a nondenaturing dot-blot assay.…”
