Validity of Midday Total Testosterone Levels in Older Men with Erectile Dysfunction

Abstract: Total testosterone in men with erectile dysfunction who are younger than 45 years should be drawn as close to 7 a.m. as possible because a statistically and clinically relevant decrease in testosterone will occur during the course of the day. Men older than 45 years with erectile dysfunction can have total testosterone drawn at any time before 2 p.m. without misleading results.

“…Fasted blood samples were collected on days 0, 14, 42, and 56 between 06:00 h and 09:00 h to limit the potential confounding effects of circadian rhythm on total testosterone in young men [17]. Blood samples were analysed for total testosterone, follicle-stimulating hormone, estradiol, sex hormone-binding globulin, luteinising hormone, insulin, cortisol, and prostate-specific antigen (Siemens Immulite 2000, Llanberis, UK).…”

“…The timing of the blood draw is important in younger men, whose testosterone levels have diurnal variation, with levels peaking early in the morning [33], but younger men were not included in this study. In older men (>45 years of age) such as most of those in our study, however, the diurnal effect on testosterone appears to be much less significant [34][35][36]. It should also be noted that the men in the UPOD study were all outpatients who were being tested for hypogonadism.…”

“…Between 7-8 AM, 5cc of blood was taken from the study subjects, by laboratory expert in the power plant dispensary, after 8-10 h fasting to measure the levels of free testosterone, LH, and FSH. 49 Blood samples were immediately placed in ice boxes and transferred to the laboratory, and their serum was separated, and stored at −20°C until the collection of all samples. 50 Serum levels of free testosterone, LH, and FSH were determined by an ELISA method using the StatFax 2100 ELISA reader at 450 nm, according to the kit manufacturer's protocol.…”
