Validation of Red Cell Distribution Width As a COVID-19 Severity Screening Tool

Abstract: Aim: The aim of this study is the predictive validation of red cell distribution width (RDW) in COVID-19 patients. Method: In total, 331 COVID-19 patients were classified as ‘severe’ and ‘nonsevere’ groups based on the WHO standard criteria. The levels of RDW standard deviation (SD) were evaluated as both continuous and categorical variables. Multivariate statistical analyses were used. Results: RDW-SD ≤43 and ≤47 fl thresholds showed high specificity (90.1–91.4%) for diagnosing nonsevere illness and no risk o… Show more

“…Another possible mechanism SARS-CoV-2 spike protein binds to the CD147 receptor that plays a role for mediating SARS-CoV-2 invasion and diffusion among other cells; the CD147 receptor is also determinative of the blood group system. The blockage of CD147 by spike protein leads to the elective trapping of RBCs in the spleen and which eventually RDW elevation [ 33 ].…”

“…Elevated RDW levels imply that the size distribution of red blood cells yields a greater variation than normal RDW values and signal bone marrow dysfunction induced by vitamin B12, folate, or iron deficiencies [10]. Pouladzadeh et al divided 331 patients with COVID-19 into 'severe' and 'non-severe' cohorts and evaluated their RDW-SD levels [11].…”

“…M2 macrophages can also play an important role in proliferation, angiogenesis, and EMT induction in tumor cells by secreting factors such as EGF, VEGF, and PDGF; it subsequently leads to their resistance to chemotherapy drugs [ 35 , 36 ]. Secreted EGF by M2 macrophages induces RAS/RAF/MAPK, PI3K/AKT and PLC/PKC/CAMK signaling in the downstream of EGF/EGFR signal transmission pathway in the tumor cells, and via binding to the EGFR receptor on the surface of cited cells; it subsequently can be effective to create survival and proliferation of malignant cells through the mentioned signaling pathways [ 37 ] (Fig.…”
