Validation of a nomogram predicting the probability of lymph node invasion based on the extent of pelvic lymphadenectomy in patients with clinically localized prostate cancer

Abstract: AB and FKHC contributed equally to this paper in 514 (65.8%), 7 in 204 (26.1%) and 8-10 in 63 (8.1%). Multivariate logistic regression models were used to test the association between predictors including PSA level, biopsy Gleason sum, clinical stage, number of nodes removed and the rate of LNI. Finally, regression coefficients were used to develop a nomogram, which was internally validated with 200 bootstrap re-samples. RESULTSThe median (range) number of lymph nodes removed was 14 (2-40); LNI was detected in… Show more

“…Previous reports have shown complication rates for limited/standard PLND in the range of 2%-9.8%, whereas extended PLND complication rates varied in the range of 19.8%-75% [8 -10]. As expected, the most frequently reported complication is lymphocele formation, with rates in the range of 4.6%-10.6% [3,10]. As would be expected, performance at high-volume centers of excellence as well as a higher level of surgical experience usually results in lower complication rates.…”

“…Similarly, Briganti et al [3] proceeded to develop and validate a nomogram for prostate cancer that predicts the probability of lymph node metastases based on clinical parameters. With the majority of patients included in that study qualifying as low-risk patients and with only 9% (a total of 71 patients) having lymph node metastases, this rather small number limits its significance and accuracy.…”

“…Meanwhile the current European Association of Urology guidelines are more moderate and recommend that patients with higher than 7% risk of LN metastases undergo PLND [4]. It is estimated that patients with D'Amico low risk prostate cancer have a risk of LN metastases of less than 7% [10]. However, the risk of LN metastases in patients with a Gleason score ≥ 7 is 25% [11].…”

“…There is debate in the literature as to who would benefit from PLND and the extent of the PLND. The 2010 National Comprehensive Cancer Network guidelines state that any patient with a 2% risk of LN metastases should undergo PLND [10]. Meanwhile the current European Association of Urology guidelines are more moderate and recommend that patients with higher than 7% risk of LN metastases undergo PLND [4].…”

“…23 A pelvic lymph node dissection should be performed during RP for high-risk prostate cancer, as 15% to 40% will be positive. 24 If positive lymph nodes are found, ADT can be commenced, which has been shown, albeit in the pre-PSA era, to improve survival. 25 High-risk patients may subsequently experience local treatment failure and require adjuvant or salvage therapies.…”
