Validation of a Head-mounted Virtual Reality Visual Field Screening Device

Abstract: Précis: The C3 fields analyzer (CFA) is a moderately reliable perimeter preferred by patients to standard perimetry. While it does not approximate the gold standard, it was sensitive and specific for clinically defined glaucoma (area under the receiving operator characteristic curve=0.77 to 0.86). Purpose: Testing the visual field is a vital sign for diagnosing and managing glaucoma. The current gold standard, the Humphrey visual field analyzer (HFA), i… Show more

“…Subjects are asked to press a button whenever they see a light target on a 2D plane extending ±30 • temporally and nasally. The concept of virtual perimetry [6] has shown increasing potential with multiple studies testing its comparability to the standard HFA [7][8][9][10][11]. Virtual reality perimetry introduces a visual grasp mode, based on eye movements instead of subjective button presses to collect the patient responses.…”

“…Although there is potential for VR devices to complement or even substitute in some cases conventional visual function tests such as perimetry, we constrained our simulated reality at least in this iteration, to a simple virtual environment that could be related to perimetric results or full-field psychophysics, while avoiding replicating a visual field test in the virtual space. 30 Likewise, for this initial design, we avoided creating visually rich scenery, which would introduce multiple, complex visual cues complicating the interpretation of the results. With this in mind, the design resulted in two sequential test paradigms.…”

“…These tests require patients to present to an office setting and should be performed by trained ophthalmic technicians [ 42 , 47 ]. There are currently a handful of tablet and virtual reality testing systems under development which may improve remote monitoring of visual fields in glaucoma patients [ 48 50 ]. Optic nerve assessment involves both optic disc photography for qualitative evaluation of the nerve and quantitative structural analysis with OCT. Standard equipment for acquiring optic nerve photos and OCTs are bulky and require in-person visits for images to be obtained.…”
