Cystic fibrosis-related diabetes (CFRD) is the principal extra-pulmonary complication of cystic fibrosis, occurring in 15-30% of adult cystic fibrosis patients. The number of cystic fibrosis patients who develop diabetes is increasing in parallel with increases in life expectancy. The aim of this study was to review the physiopathology, clinical presentation, diagnosis and treatment of CFRD. A bibliographic search of the Medline and Latin American and Caribbean Health Sciences Literature databases was made. Articles were selected from among those published in the last twenty years. Insulin deficiency, caused by reduced beta-cell mass, is the main etiologic mechanism, although insulin resistance also plays a role. Presenting features of type 1 and type 2 diabetes, CFRD typically affects individuals of approximately 20 years of age. It can also be accompanied by fasting, non-fasting or intermittent hyperglycemia. Glucose intolerance is associated with worsening of nutritional status, increased morbidity, decreased survival and reduced pulmonary function. Microvascular complications are always present, although macrovascular complications are rarely seen. An oral glucose tolerance test is recommended annually for patients > or = 10 years of age and for any patients presenting unexplained weight loss or symptoms of diabetes. Patients hospitalized with severe diseases should also be screened. If fasting hyperglycemia persists for more than 48 h, insulin therapy is recommended. Insulin administration remains the treatment of choice for diabetes and fasting hyperglycemia. Calories should not be restricted, and patients with CFRD should be managed by a multidisciplinary team.

Physical activity and quality of life of children and adolescents with cystic fibrosis: a cross-sectional study

Introduction: Regular physical activity (PA) is part of outpatient care offered to most people with cystic fibrosis (CF). Objective: To determine the frequency of the self-reported PA and, secondarily evaluate associations between PA with clinical variables and health-related quality of life (HRQoL) in pediatric and adolescent CF subjects. Method: In this cross-sectional study, subjects between 6 and 17 years of age with a confirmed diagnosis of CF were recruited in the outpatient clinic. The subjects answered questions about the practices of PA, HRQoL questionnaire and clinical variables were collected. Subjects were classified into two groups according to weekly PA practice: PA performed ≥ 3 times/week and performed PA ≤ 2 times/week. Results: 66 subjects completed the study, 72.7% (n=48) had PA ≥ 3 times/week and 27.2% (n=18) had PA ≤ 2 times/week. Only twelve children (18,2%) reached the recommendations of the World Health Organization (WHO) to practice moderate to vigorous physical activities daily. The mean age of the subjects evaluated was 12.3 ± 3.2 years, forced expiratory volume in one second 90 ± 24.1% and forced vital capacity 95 ± 20.4%. The group that performed PA ≥ 3 times/week had a better clinical score (p=0.033), a lower number of hospitalizations in the year (p=0.002), a lower number of days hospitalized in the last year (p=0.020) and better score for the physical (p=0.003) and emotion (p=0.048) domains in HRQoL questionnaire. Conclusion: This study concluded that most subjects did not reach the WHO recommendations for the practice of PA. However, individuals who practice PA at least 3 times/week have better HRQoL, clinical score and fewer hospitalizations.

OBJETIVO: Avaliar a qualidade de vida (QV) dos pacientes com fibrose cística (FC) acompanhados por um centro de referência de Minas Gerais mediante a aplicação de questionário específico e validado.MÉTODOS: Estudo transversal, de análise quantitativa, com aplicação do questionário de QV em FC, escore de Shwachman-Kulczycki e valores espirométricos. A amostra do estudo foi composta por 26 pacientes com idade de 6 a 29 anos e 13 pais de crianças com idade de 6 a 13 anos. Os participantes foram divididos em três grupos: grupo I (6 a 13 anos), grupo II (pais ou cuidadores de crianças de 6 a 13 anos), grupo III (≥14 anos). Os escores foram apresentados por médias e medianas e analisados por meio do teste de correlação linear de Pearson. Foi adotado o nível de significância de 5%.RESULTADOS: Os pacientes com FC acompanhados neste hospital de referência de Minas Gerais apresentavam boa percepção de QV e escore de Shwachman-Kulczycki excelente. No grupo I, foram encontrados valores menores para o domínio digestório (67,49%) e maiores para o domínio execução de tratamento (83,84%). No grupo II observou-se menores pontuações no domínio peso (65,70%) e maiores na imagem corporal (84,61%). No grupo III, valores menores para o domínio social (71,12%) e maiores para alimentação (92,47%). Com relação aos valores espirométricos, encontrou-se valores significativos correlacionando a relação entre o volume expiratório forçado no primeiro segundo e a capacidade vital forçada (VEF1/CVF) aos domínios físico/percepção da saúde e funcionalidade no grupo III.CONCLUSÕES: Os pacientes com FC acompanhados no centro de referência deste estudo apresentaram boa percepção de QV, o que pode ser verificado, na maioria dos domínios analisados, por escores superiores a 50. O uso de questionários específicos para avaliação de QV é um método de baixo custo e de fácil aplicação, devendo ser verdadeiramente incluído à rotina dos serviços de saúde que atendem a população diagnosticada com FC.
