Vaginal prostaglandin gel to induce labour in women with one previous caesarean section

Abstract: This retrospective study reviewed the mode of delivery when vaginal prostaglandins were used to induce labour in women with a single previous lower segment caesarean section. Over a 4-year period, PGE 2 gel was used cautiously in low doses in 54 women. Induction with PGE 2 gel was associated with an overall vaginal birth after caesarean section (VBAC) rate of 74%, which compared favourably with the 74% VBAC rate in women who went into spontaneous labour (n = 1969). There were no adverse outcomes recorded after… Show more

“…In our study the rate of uterine rupture after prostaglandin induction of labour with or without previous Cesarean was 2.5% and 0.033%, respectively. This is similar to the data shown by other investigators [10][11][12][13][14][15][16]. The risk of uterine rupture is influenced by many factors including birth weight, gestational age, race, country and maternal characteristics [17,18].…”

“…Our results show a lower uterine rupture rate compared with studies using prostaglandins for induction of labour. Some authors have described uterine rupture rates with dinoprostone induction between 0 and 5.9% [6, 18]. Our results revealed a rate of uterine rupture similar to other studies about mechanical methods in cervical ripening [19, 20].…”

“…Moreover, we could not include in our review any studies using mechanical IOL (Foley or double balloon catheter). In addition, sequential administration of oxytocin or amniotomy performed to augment labor might increase the risk of complications in the IOL group [16,17]. Therefore, duration of labor might play a role in the efficacy of IOL, because a long labor may require more augmentation than a short labor, which in turn might increase the risk of complications.…”
