Abstract:Purpose
We sought to evaluate patient adherence and response to simple vaginal and sexual health treatment strategies in female cancer patients receiving treatment at a female sexual medicine and health program, and identify improvements of physical symptoms, per patient and clinical evaluation.

Resumo: Objetivo: Comparar a contração objetiva do assoalho pélvico, estenose vaginal e dispareunia com a função sexual e a qualidade de vida relacionada à saúde de sobreviventes ao câncer ginecológico. Método: Estudo transversal com mulheres com diagnóstico de câncer ginecológico e com no mínimo seis meses de término do tratamento oncológico. A avaliação consistiu na aplicação da ficha de identificação, questionários (QS-F e FACT-G) e exame físico ginecológico. Resultados: Foram avaliadas 64 pacientes com média de idade de 50 anos, das quais 56,3% eram ativas sexualmente. O tipo de câncer mais frequente foi o de colo uterino (62,5%), sendo o principal tratamento realizado a histerectomia (54,7%). Observou-se a presença de dispareunia em 32,8% das mulheres ativas sexualmente, a estenose vaginal foi identificada em 45,3% dos casos, e 45,3% apresentaram contração objetiva ausente do assoalho pélvico. A comparação das médias dos escores do questionário QS-F evidenciou diferença significante considerando a presença ou não de estenose vaginal e dispareunia, enquanto que a presença ou ausência da contração objetiva do assoalho pélvico não apresentou diferença significante na função sexual. Verificou-se associação entre estenose vaginal e dispareunia, porém a atividade sexual e a contração do assoalho pélvico não estiveram associadas à estenose vaginal. Além disso, a estenose vaginal, dispareunia e a contração do assoalho pélvico não apresentaram associação significante com a qualidade de vida. Conclusão: A presença de disfunção do assoalho pélvico nas sobreviventes ao câncer ginecológico indicou prejuízo na função sexual, sem impactar negativamente na qualidade de vida relacionada à saúde.

Sexual dysfunction (SDF) is a common sequel to cancer treatment which affects the quality of life in women treated with pelvic radiotherapy. The aim of this study was to evaluate the safety, symptom resolution and objective improvement the injection of autologous platelet released growth factor (APRGF) for treatment of SDF in cited patients. This prospective pilot study enrolled 10 cancer-free patients with SDF who underwent pelvic radiotherapy at least 5 years ago, randomly. Each patient was received 1-2 cc APRGF within four weeks and all patients were re-evaluated at eight weeks and six months. CD34 immuno histochemistry and Masson’s trichrome staining were performed on vaginal biopsy section for angiogenesis and fibrosis assay respectively. Sexual satisfaction after the injection of APRFG was clinically difference and the entire patient had sexual satisfaction. In the patient’s follow-up, none of them needs to repeat the treatment. Our results declared that APRGF injection was effective and symptoms were disappeared in the entire patients. Significant objective improvements in vaginal diameter (mean before injection, 6.5 cm vs 7.1 cm after injection) (p-value = 0.001) and vaginal flexibility (mean before treatment, 0.72 cm vs 1.85 cm after injection) (P-value = 0.026) were observed. Characteristics of discharge before the injection in 60% of patients were included dry vagina and 40% had mild discharge but after injection 40% of patients had moderate and also 60% had mild and sufficient discharge (P-value= 0.190). Overally, our patients reported better sexual function and showed better vaginal function indexes, after APRFG injection.
PurposeSexual health concerns are common in breast cancer surgery but often overlooked. Yet, breast cancer patients want more sexual health information from their providers. We aimed to share ways for providers to address sexual health concerns with their breast cancer patients at different stages of the treatment process.MethodsExperts in breast cancer treatments, surgeries, and sexual health at Memorial Sloan Kettering Cancer Center assembled to review the literature and to develop the recommendations.ResultsProviders should provide sexual health information for their breast cancer patients throughout the continuum of care. Conversations should be initiated by the providers and can be brief and informative. Whenever appropriate, patients should be referred to Sexual Medicine experts and/or psychosocial support. There are various recommendations and tools that can be utilized at diagnosis, endocrine and chemotherapy, and breast surgery to identify patients with sexual health concerns and to improve their sexual functioning.ConclusionIn this paper, we sought to provide providers with some insights, suggestions, and tools to address sexual health concerns. We encourage healthcare providers to initiate the conversation throughout the continuum of care beginning as early as diagnosis and refer patients to additional services if available.
