Background: To identify new markers of juvenile systemic lupus erythematosus (JSLE) that facilitate patient stratification and prognosis is quite important. Therefore, our aim of the present study is to analyze alteration of protein expression and potential valuable biomarkers in juvenile systemic lupus erythematosus (JSLE) urine. Methods: Based on this aim, proteomics assay analyzed the changes of urinary proteins in study groups consisting of 9 healthy controls, 9 inactive JSLE and 10 active JSLE patients. And the correlationship between clinical characteristics of JSLE patients and new biomarkers discovered from proteomics assay was qualified.Results: We have identified a group of 105 differentially expressed proteins with ≥1.3 fold up-regulation or ≤0.77 fold down-regulation in JSLE patients. In gene ontology and functional enrichment analysis, we discovered these proteins were involved in several important biological processes such as acute phase inflammatory responses, complement activation, hemostasis, and immune system regulation. Interestingly, we found that Ephrin type-A receptor 4 (EPHA4) and Vitronectin (VTN) were significantly reduced in the urine of both inactive and active JSLE patients. Especially, the urinary VTN was also linear correlated with clinical characteristics of JSLE. Conclusion: In summary, this study provided a reliable proteomic reference profile for JSLE. Patients with active and inactive JSLE have differentially obvious metabolic proteins in the urine. VTN could be a specific diagnostic biomarker to distinguish inactive and active JSLE. These identified proteins as new biomarkers in this study can warrant some further exploiting in a larger prospective validation study.

In modern rheumatology, the problem of differential diagnosis of bacterial infection and active rheumatic process still retains its relevance. At the same time, it is very important to search for a biomarker - the gold standard for the diagnosis of an infection in patients with rheumatic diseases (RDs) in order to rapidly determine a treatment policy. This review analyzes the diagnostic significance and possibility of using some laboratory markers for bacterial infections in modern rheumatology. It emphasizes the importance of a multimarker approach that allows increasing the significance of individual parameters in the diagnosis of infections in RD.

Auto-immune complexes are an important feature of several auto-immune diseases such as lupus, as they contribute to tissue damage through the activation of immune cells. Neutrophils, key players in lupus, interact with immune complexes through Fc gamma receptors (FcgR). Incubation of neutrophils with aggregated-IgGs increased the surface expression of FcgRI within minutes, in a concentration-dependent fashion. After 30 min, IgG aggregates (1 mg/ml) up-regulated FcgRI by 4.95 ± 0.45-fold increase. FcgRI-positive neutrophils reached 67.24% ± 6.88% on HA-IgGs stimulated neutrophils, from 3.12% ± 1.62% in non-stimulated cells. Compared to factors reported to up-regulate FcgRI in neutrophils, IgG-aggregates were most potent. FcgRII, and possibly FcgRIIIa, appeared to mediate this up-regulation. Syk and PI3K were involved in this process.Moreover, FcgRI-dependent signaling was found to be necessary for ROS production in response to IgG-aggregates. Finally, combinations of aggregates with LPS or fMLP dramatically boosted ROS production. Co-engaging FcgRI and other signaling pathways can impact neutrophil inflammatory responses in a major fashion. This work reveals FcgRI as an essential component in the response of human neutrophils to immune complexes and may help explain how they contribute to tissue damage associated with immune-complex diseases, such as lupus.
