Puerperal uterine inversion is a severe but rare obstetric complication of yet unknown origin. In this two-year study we determine the incidence of this complication and we describe associated risk factors to expose its etiology. All cases of uterine inversion were included from a nationwide cohort study which contained all 98 hospitals with a maternity unit in the Netherlands. We reviewed the medical records of 15 patients, resulting an incidence of approximately 1 in 20 000 vaginal births. Fourteen cases (93.3%) were classified as low-risk pregnancies at booking. Nulliparous women were not overrepresented and the main associated factors were signs of prolonged labor followed by third stage manipulation. This study is the first population-based study for uterine inversion. With the reported associated factors and occurrence in women with a low-risk profile, we show that every birth attendant should be able to detect this rare but severe complication.

ZusammenfassungDie puerperale Uterusinversion ist eine seltene aber lebensbedrohliche geburtshilfliche Komplikation. Nicht erkannt kann es zu schweren Blutungen und Schock bis hin zum mütterlichen Tod führen. Eine manuelle Reposition unter Vollnarkose ist meistens erfolgreich. Bei resistenten Inversionen kann eine Korrektur per Laparotomie hinzugezogen werden. In jedem Fall können die Mortalität und Morbidität durch rechtzeitige Erkennung und zügige Reposition vermindert werden. Daher sollten regelmäßige praktische Übungen zur Erkennung und Behandlung der akuten uterinen Inversion, wie auch andere geburtshilfliche Notfälle im Rahmen der Facharztausbildung erfolgen. Wir berichten über eine 31-jährige II. Gravida 0. Para in der 39+4. Schwangerschaftswoche (SSW), bei der eine akute Inversio uteri dritten Grades nach einer spontanen Entbindung aufgetreten war, die bei frustraner vaginaler Reposition per Laparotomie in Intubationsnarkose reponiert wurde.

Two cases of puerperal inversio uteri are reported. The literature has been briefly reviewed and the aetiology as well as the frequency of inversio uteri has been discussed. In most cases the inversion is spontaneous, but a usual cause of inversio uteri is supposed to be too heavy expression of the afterbirth at the third stage of the delivery when the placenta is not loosened and the uterus is not contracted. The parity is of no importance. The first symptom usually is shock which not always is proportional to the bleeding. Also fibrinolytic blood coagulation defects are mentioned. The diagnosis is evident when the uterus is found in or outside the vagina. The treatment is manual reposition as soon as possible after the shock has been treated. The two reported cases are both puerperal inversions. The first, para 2, was admitted in shocked condition. The reposition was easy and no complications occured, while the other case was a para 4 complicated with a fibroma of the uterus. This patient died after the reposition was made as it was not possible to treat the shock adequately.
