This paper presents the case study of a 74 year old man with liver cirrhosis and consumptive syndrome. Data was collected in a Brazilian university hospital. The case study approach used was outcomepresent state test (OPT) model. The assessment was made by using the Conceptual Model of Wanda Horta. Also, the NANDA-I, Inc., Nursing Outcomes Classification (NOC), and Nursing Interventions Classification (NIC) linkages were used to capture accurate nursing care plan. The keystone nursing diagnosis was: Imbalanced nutrition: less than body requirements. The NOC outcomes chosen were Nutritional Status: Food and Fluid Intake; and Weight: Body Mass Control. The NIC interventions chosen for the patient were: Enteral Tube Feeding and Fluid Monitoring. The findings support that the use of a clinical reasoning web helps nurses to identify the priority focus of care in patients with complex health problem, and to prevent future complications associated with the evolution of disease. Future researches are needed to determine the NANDA-I, NOC, and NIC linkages based on a clinical reasoning model to capture accurate nursing care plan for all patients with chronic condition, in order to provide competent and safe care, thus improving patient outcomes.

RESUMOTrata-se de estudo de caso de paciente idosa, internada em unidade médico-cirúrgica por insuficiência cardíaca descompensada, que teve como objetivo identificar, por meio do modelo de raciocínio clínico Outcome Present State Test (OPT), os diagnósticos, resultados e intervenções de enfermagem. A coleta de dados foi realizada por meio de instrumento previamente validado e direcionado às condições cardiológicas. Os diagnósticos, resultados e intervenções foram realizados com o uso de sistemas padronizados de linguagem NANDA-I Inc., Nursing Outcomes Classification e Nursing Interventions Classification. O diagnóstico central foi débito cardíaco diminuído. Os resultados e as intervenções escolhidos foram relacionados aos estados cardíacos e respiratórios, sobrecarga hídrica, ansiedade, fadiga, perfusão tissular periférica, conhecimento e autocontrole da insuficiência cardíaca, sinais vitais e melhora do sono. A terceira geração do processo de enfermagem, representada pelo OPT, auxiliou na identificação das principais necessidades do paciente e direcionou o planejamento da assistência com base nas prioridades do cuidado.

RESUMENIntroducción: El proceso de enfermería es un método que involucra conocimientos, habilidades, destrezas, capacidades, actitudes y valores adquiridos durante la formación profesional, por tanto es fundamental garantizar el logro de esta competencia en la práctica clínica. El objetivo consistió en evaluar el nivel de cumplimiento y alcance de las competencias en Proceso de Enfermería en los estudiantes de Enfermería. Materiales y Métodos: Diseño de corte transversal, cuya muestra correspondió al total de estudiantes matriculados (73 estudiantes) en IV y VI nivel de un programa de enfermería de la ciudad de Bucaramanga que rotaban por áreas clínicas y comunitarias. Resultados: Según la opinión de los estudiantes, las competencias de proceso de enfermería fueron alcanzadas entre 2.63% a un 100% y un nivel de cumplimiento de 2.7 y 4, que según la escala establecida están entre regular y muy bueno. La competencia "Brinda cuidado aplicando el proceso de enfermería" fue común entre rotaciones de IV nivel y no se encontraron diferencias en el nivel de logro percibido (p=0.3180 test ANVOVA), además evidenció una diferencia significativa entre el nivel alcanzado entre IV y VI semestre. Discusión: Aunque existen pocos estudios sobre la medición de las competencias en proceso de enfermería durante la formación universitaria, un gran número de publicaciones científicas han hecho evidente los resultados de la aplicación de este método en la práctica clínica. Conclusiones: El conocer en qué medida las competencias en proceso de Enfermería son alcanzadas y en qué nivel, facilita el ajuste de aspectos curriculares del programa del estudio.
