Using humeral nail for surgical reconstruction of femur in adolescents with osteogenesis imperfecta

Abstract: Osteogenesis imperfecta (OI) is a rare inherited connective tissue disorder caused by mutation of collagen which results in a wide spectrum of clinical manifestations including long bone fragility fractures and deformities. While the treatment for these fractures was recommended as using intramedullary fixation for minimizing stress concentration, the selection of the best implant in the adolescent OI patients for the surgical reconstruction of femur was still problematic, due to anatomy distortion and implant… Show more

“…Intramedullary fixation is often recommended for fracture repair. 3 , 6 8 For this patient, as the owner and veterinarian attempted a different approach, K-wires were used for fracture repair of the long bones. K-wires were also inserted into the intact long bones to distribute stress.…”

“…Preventive intramedullary pinning can also be attempted to distribute the load on long bones. 6,7 However, owing to poor prognosis, euthanasia is often elected in veterinary medicine. 1 Thus, this case report aims to describe the long-term Long-term follow-up of a cat with an undetermined osteoporotic bone disease managed with multiple intramedullary pins management of suspected OI by surgical repair using intramedullary pins in a cat.…”

“…Sangasoongsong et al support the view of using humeral nails in femoral fixation in patients with OI over Rush nails as they have a smaller diameter and provide the interlocking property which is better for rotational stability [19]. Mulpuri and Joseph report the results of a 10-year period of intramedullary rodding in 16 patients.…”

“…Pre-operative planning and selection of the best implant for patients with OI is very important but problematic as well. The age of the patient, the distortion of the anatomy, the advantages and disadvantages of the various surgical instruments, the availability of the different surgical instruments, the surgical experience, and the post-operative complications are all factors that should be carefully examined in order to select the best implant for each case [18,19]. With our case report, we present a relatively simple surgical technique for correction of long bone deformities that can be a good alternative when there is no access to advanced, innovative surgical systems, such as telescoping rods.…”

“…However, the common orthopaedic manifestations seen in the OI patients, due to excessive fragility are frequent fractures, progressive deformity of long bones and impaired ambulation. 2,3 The treatment of OI requires a multidisciplinary approach includes both medical and surgical treatment. Medical management includes use of bisphosphonates therapy to increase bone mineral density and reduce long bone fractures rate and surgical management for fracture fixation or deformity correction.…”
