Background: PCR assay has provided a mean of more rapid and sensitive detection of Mycobacterium tuberculosis (MTB) complex than conventional acidfast bacilli (AFB) smears and MTB cultures. Using the recently developed AdvanSure TB/NTM kit (LG Life Science Diagnostic Division, Korea), which could differentiate nontuberculous mycobacteria (NTM) from MTB, this study compared clinical usefulness of real-time PCR assay and Amplicor MTB PCR assay (Roche Molecular Systems, USA) for diagnosis of tuberculosis. Methods: A total of 213 specimens (148 respiratory and 65 nonrespiratory specimens) were tested by using real-time PCR, Amplicor MTB PCR, AFB smear, and MTB culture. The sensitivity and specificity of four methods were evaluated according to clinical diagnosis. Results: Of six NTM grown in culture, four (67%) were detected by real-time PCR. The overall agreement of real-time and Amplicor MTB PCR was 92% (191/207). The overall sensitivity and specificity were 91% and 87%, respectively, for real-time PCR, and 86% and 93% for Amplicor MTB PCR. In nonrespiratory specimens, the sensitivities of real-time PCR, Amplicor MTB PCR, AFB smear, and MTB culture were 67%, 60%, 13%, and 40%, respectively, and the specificity of the four methods were all 100%. Conclusion: For diagnosis of tuberculosis, the sensitivity and specificity of the real-time PCR assay using AdvanSure TB/NTM kit and Amplicor MTB PCR were similar, and the former could differentiate NTM from MTB. The PCR assay can be considered as a more sensitive technique for the detection of MTB than the conventional AFB smear and culture. (Korean J Clin Microbiol 2008;11:29-33) 29 서 론 결핵은 세계적으로 가장 중요한 전염성 질환 중의 하나로 매 년 800만명이 발병하고 300만명이 사망하는 주요한 보건 문제 이다[1]. 근래 들어 후천성 면역 결핍 증후군이 증가하고 다제 내성 결핵균이 등장하면서 결핵 빈도가 증가하고 폐외결핵도 문제가 되고 있다. 한국에서 결핵은 감소하는 추세이나 외국에 비해 유병률이 높은 편이고[2], 과거에는 항산균 양성이면 대부 분 결핵으로 간주했으나 최근 비결핵항산균(nontuberculous mycobacteria, 이하 NTM)이 증가하고 있어 이를 감별할 필요 가 있다[3-5]. 결핵의 진단에는 보편적으로 항산균 도말 검사와 배양 검사 를 이용해 왔다. 항산균 도말 검사는 신속하고 간편하다는 장 점이 있으나 민감도와 특이도가 떨어진다는 단점이 있고, 배양 검사는 결핵 진단의 표준법으로 가장 정확한 방법이지만 시간 이 오래 걸린다는 단점이 있다. 최근 면역과 분자진단 기술이 발달되어 이를 이용한 결핵의 진단법들이 많이 개발되었다 [6-8]. 그 중에서 핵산을 증폭하는 기술 중의 하나인 중합효소 연쇄반응(polymerase chain reaction, 이하 PCR)을 이용하여 검 체에서 직접 결핵균을 검출하는 방법은 신속하면서 민감도가 높아서 널리 이용되고 있다. 표준화된 방법을 사용하여 객관적 인 결과를 제공할 수 있으며 호흡기 검체뿐 아니라 다양한 검 체에서 민감도가 높은 것으로 알려져 있다.
