Introduction: Dermatological access in rural regions has been impacted due to an acute, global dermatologist shortage coupled with a striking disparity in dermatologist density between urban and rural areas. As a result, the dermatological arena has been under notable pressure to amplify access. Teledermatology has entailed the use of technology to provide dermatological services to individuals located at a remote distance. The purpose of this literature review was to examine the effect of utilization of teledermatology to determine enhancement of dermatological access to residents of rural areas. Materials and Methods: This review followed a systematic approach and utilized five electronic databases to obtain peerreviewed journal articles. A PRISMA approach was used and a total of 86 references were employed. Results: Teledermatology programs have been able to complement conventional dermatological care to enhance dermatological access to rural areas that have suffered from a shortage of dermatologists and could aid in supplementing traditional care as well. Within rural settings, the results of three studies in this review indicated the importance of improved quality for diagnostic precision, whereas one study reported that clinical images might not provide sufficient insight to deliver clear-cut diagnoses. In addition, enhancements in diagnostic precision could be obtained by upgrades in phone cameras to capture images. Finally, to most of the existing literature, in using teledermatology, physician satisfaction has been stronger than patient satisfaction. Conclusions: Teledermatology has had a beneficial impact in improving dermatological access to rural areas. The success of this technology is contingent upon the commitment and willingness of the dermatologist in utilizing it.

To assess the impact of live interactive teledermatology consultations on changes in diagnosis, disease management, and clinical outcomes. Design: We conducted a retrospective analysis of 1500 patients evaluated via live interactive teledermatology between 2003 and 2005 at the University of California, Davis. We compared diagnoses and treatment plans between the referring physicians and the teledermatologists. Patients with 2 or more teledermatology visits within a 1-year period were assessed for changes in clinical outcomes. Setting: Academic medical center with an established teledermatology program since 1996. Participants: Medical records were evaluated for 1500 patients who underwent live interactive teledermatology consultation. Patients seen for more than 1 teledermatology visit were included in the clinical outcome assessment. Intervention: Live interactive teledermatology consultation. Main Outcome Measures: Changes in diagnosis, disease management, and clinical outcome. Results: Compared with diagnoses and treatment plans from referring physicians, the 1500 live interactive teledermatology consultations resulted in changes in diagnosis in 69.9% of patients and changes in disease management in 97.7% of patients. Among 313 patients with at least 2 teledermatology visits within 1 year, clinical improvement was observed in 68.7% of patients. Multivariate analysis showed that changes in diagnosis (P = .01), changes in disease management (P Ͻ .001), and the number of teledermatology visits (P Ͻ .001) were significantly associated with improved clinical outcomes. Conclusions: Live interactive teledermatology consultations result in changes in diagnosis and disease management in most consultations. The numbers of live interactive teledermatology visits and changes in diagnosis and disease management are significantly associated with improved clinical outcomes.

Amaç: Teledermatoloji, dermatolojik bilgilerin telekomünikasyon teknolojileri kullanılarak uzak mesafelere iletilmesidir. Bu çalışmada, ülkemizde dermatologların teledermatoloji hakkındaki bilgi düzeyini, günlük klinik pratiklerinde kullandıkları teledermatoloji yöntemlerini ve teledermatoloji ünitelerinin kurulması durumunda sergileyecekleri yaklaşımları değerlendirmeyi amaçladık. Yöntem: Tanımlayıcı kesitsel olarak planlanan bu çalışmada, 15 sorudan oluşan anket elektronik posta ile dermatologlara iletildi. Sekiz haftalık bir süre içerisinde tam ve hatasız doldurularak geri gönderilen anketler çalışmaya dahil edildi. Bulgular: Yüz on altı anketin değerlendirmeye alındığı çalışmada, katılımcıların %37.9'unun üniversite, %28'inin eğitim ve araştırma, %27'sinin devlet, %18.1'inin de özel hastanelerde çalıştığı gözlendi. Öte yandan katılımcıların %94'ünün, çalıştıkları kliniklerde teledermatoloji ünitesinin olmadığı, %50'sinin teledermatoloji hakkında az, %25.9'unun ise orta düzeyde bilgi sahibi olduğu saptandı. En sık (%43.10; 50/116) kullanılan teledermatolojik yöntemin dermatologlara elektronik posta yoluyla hasta resmi gönderilerek ("depola ve gönder") "danışma" şeklinde yapıldığı öğrenildi. Teledermatolojinin ülkemizde resmi olarak kurulup uygulanmasını dermatologların %55.2'si "çok fazla", %30.2'si ise "fazla" istediğini belirtti. Sonuç: Bu çalışmayla, birçok meslektaşımızın teledermatoloji konusundaki bilgileri sınırlı olmasına karşın, birçoğunun mesleklerini icra ederken teledermatolojiyi kullanabildikleri ve ülkemizde resmi olarak uygulamaya konmasına sıcak baktıkları ortaya konulmuştur.
