Use of the intubating laryngeal mask in children: an evaluation using video-endoscopic monitoring

Abstract: The size 3 intubating laryngeal mask provides an airway that is easy to establish in children > or = 25 kg with excellent ventilation conditions and allows blind tracheal intubation at the first attempt with a high success rate. Endoscopic monitoring improves its safety and intubation success rate.

“…Additionally our results for supine patients are quite comparable with light-wand guided intubation via ILMA in supine patients [11-14]. Fibreoptic guidance definitely improves success rates as reported by Joo and Rose [15], where he compared blind versus fibreoptic intubation via ILMA and by Weiss et al [16] who used fibre-optic guidance in pediatric patients after a failed blind intubation.…”

“…There are a number of different techniques reported in the literature to aid SAD insertion: with bougie (64), with laryngoscope (65), rotational insertion (66,67), part inflated (68), laterally inserted part inflated (69), and blind (70). Most of these relate to the classic laryngeal mask airway, but there is some limited evidence that the ProSeal TM is of use in children (24,70,71) and the Intubating Laryngeal Mask Airway, in children over 8 (42).…”

“…This technique should be practiced under controlled conditions or on a mannequin before attempting this in an emergency situation. Although successfully used in adults by highly experienced users, blind insertion of the tracheal tube through the LMA ⁄ iLMA should be avoided (36).…”
