Traumatic brain injury (TBI) is one of the leading causes that cognitive and behavioral problems. Many pharmacological therapies reveal early neurochemical changes. The CRS-R is a standardized neuro-behavioral evaluation tool comprising six organized subscales (auditory, visual, motor, oromotor-verbal, communication, and arousal). We hypothesized that, the effect of amantadine sulfate infusion treatment on conscious recovery and clinical improvement in patients with severe head injury in terms of CRS-R and clinical recovery. M Ma at te e-r ri ia al l a an nd d M Me et th ho od ds s: : Patients' age, gender, days of hospitalization, pre-treatment and post-treatment GCS, education level, day of amantadine sulfate, intubated or non-intubated when he/she come to intensive care unit (ICU), recovery time of conscious, discharge time, CT or MR pathology will be retrospectively recorded. Patients were aged >18 years, had brain trauma, had amantadine sulfate therapy in ICU were included the study between June 2016-June 2017. R Re es su ul lt ts s: : A total of 40 patients using amantadine sulfate infusion were included in the study. It was determined that 13 of patients had mortality. Improvement in CT and MRI findings were detected in 21 of the living patients (77.8%). There were no differences between the mean starting time according to the clinical recovery levels and recovery status on CT-MRI. There were statistically significant differences between starting time and time of extubation and starting time of consciousness recovery and discharge time. Percheron artery syndrome is characterized by infarction at the paramedian nucleus of the thalamus and mesencephalon. Its classical triad comprises altered mental status with various clinical findings ranging from confusion to coma, restricted vertical gaze, and impaired memory. In this study, three cases of occlusion of the Percheron's artery were presented along with their clinical and magnetic resonance findings. Their treatment additionally included infusion of amantadine, and its positive effect on the prognosis was assessed. GirişTalamus; beynin hem ön hem de arka sistemden çok sayıda küçük dallar aracılığıyla beslenmektedir. Arka damar sisteminde yer alan posterior kommunikan arter, posterior serebral arterin P1 ve P2 dalları talamusun beslenmesinde ön plandadır.Percheron arteri; Gerard Percheron (1) tarafından 1973 yılında tanımlamıştır. Percheron arteri; talamoperforan arterin azygos dalı-dır. Percheron arter tıkanması; talamusun paramedian nukleusu ve mezensefalonda infarktla sonuçlanır. Otopsi çalışmalarında Castaigne ve ark.ları (2) 4 olguda mezensefalon, red nukleus, intralaminar, parafasiküler yapılar ve talamusun santral ve median nukleuslarının tutulumu ile ilişkili sendrom olarak göstermişlerdir. Mental durum değişiklikleri, vertikal bakış kısıtlılığı ve hafıza bozukluğu klasik triadı oluşturmaktadır (1, 2). Klinik ve görüntüleme yöntemleri ile Percheron arter tıkanıklığı olan olguların ayırıcı tanısı yapıldı. Aynı zamanda her üç olguya bilinçteki değişiklikler ve uykuya eğilimleri nedeni ile tedavide NMDA (N-metil-D-Aspartat) reseptör antagonisti olan amantadin kullanılarak klinik takipleri yapılmış, prognoza olan etkisi değerlendirilmiştir. Olgu SunumlarıOlgu 1 Altmış yedi yaşında bayan hasta; 3 gün önce ani başlayan baş ağrı-sı, bilinçte uykuya eğilim ve sol göz kapağında düşme olması nedeni ile kliniğimiz yoğun bakım ünitesine yatırıldı Yatışı sırasında fizik muayene olağandı, tansiyon arteriyel (TA) 110/60 mmHg, nabız 86/dk, ateş 36,2˚C idi.Nörolojik muayenesinde bilinç uykuya eğilimli, verbal iletişimi, yer, zaman, kişi oryantasyonu kısıtlı idi. Pupiller izokorik, bilateral direkt ve indirekt ışık refleksi azalmış, solda belirgin bilateral pitozu, bilateral vertikal bakış paralizisi mevcuttu. Ense sertliği, meninks irritasyon bulguları olmayan olgunun, ekstremiteler arasında asimetrisi yoktu, spontan hareketi mevcuttu. Patolojik refleksleri ve ilkel refleksleri negatif, derin tendon refleksleri normoaktif saptandı. Kooperasyon kısıtlılığı nedeni ile serebellar ve duyu muayenesi yapılamadı. Yatış sırasında NIH inme skalası 4 idi (Tablo 1).Hastanın yakınlarından alınan anamnez ve yapılan nörolojik muayenesi ile iskemik inme, hemorajik inme, ansefalit tanıları düşünüldü.

Amantadine is a dopaminergic agent with possible N-methyl-D-aspartate antagonist effects. Dopaminergic agents have been succesfully used for many purposes, including prophylaxis for influenza, adjunctive therapy for psychiatric diseases, treatment for Parkinson Disease, and treatment for disorders of consciousness. In this case report, amantadine infusion was started in a patient whose unconciousness could not be identified for ten days. A dramatic recovery was detected on the second day following amantadine infusion. The patient was discharged on the twenty-first treatment day with a 15 GCS score. Conclusion: Amantadine may be added to the standard therapy in comatose patients as it accelerates neurological functional recovery.
