Use of technology for the objective evaluation of scratching behavior: A systematic review

Abstract: Introduction: Pruritus is a common symptom across various dermatologic conditions, with a negative impact on quality of life. Devices to quantify itch objectively primarily use scratch as a proxy. This review compares and evaluates the performance of technologies aimed at objectively measuring scratch behavior. Methods: Articles identified from literature searches performed in October 2020 were reviewed and those that did not report a primary statistical performance measure (eg, sensitivity, specificity) were e… Show more

“…Objective measures of pruritus focus on binary classification of scratching behaviors, which indicate whether a patient is scratching or not [14]. Tracking of these scratching behaviors provides insight into the effect of itch on an individual over time.…”

“…Tracking of these scratching behaviors provides insight into the effect of itch on an individual over time. Video recording using cameras (sometimes infrared) is the current standard in many studies [14,15]. However, this method is time intensive as it requires manual labeling of scratching events by researchers.…”

“…Many of these wearable sensor studies explore nocturnal scratch detection from wrist-worn actigraphy or smartwatch applications. Yang et al 9 provide a systematic review of studies published before 2021 to compare methods of measuring scratch objectively and pointed out the large variability in performance and limited evaluation of specificity due to imbalanced data. In addition, lack of generalizability and failure to capture finger scratches would bring additional errors.…”

“…During periods of rest and sleep, the willful regulation of behaviors subsides, and other regular daily movements do not interfere with scratching movements. That is why nocturnal scratching has been recognized as a promising novel measurement [ 10 ] that brings new insight complementary to current AD measures, offering an opportunity for continuous measurements out of the clinic but also with a certain degree of objectivity. Digital health technologies (DHTs) allow researchers of nocturnal scratch and AD to collect and assess quantifiable information about the behavior of scratching and its impact on sleep and, hence, enhancing our toolbox of assessments to uncover complex relationships between itch, scratching, and sleep, as well as the changes induced by treatments [ 11 ].…”
