Use of Dynamic Two-Dimensional Transesophageal Echocardiography for Renal Cell Carcinoma with Cavoatrial Tumor Thrombus

Abstract: Renal cell carcinoma with inferior vena caval tumor thrombus extending to the level of the right atrium occurs in about 1 % of all cases. Dynamic two-dimensional transesophageal echocardiography is a minimally invasive safe technique that can demonstrate preoperatively the cephalad extent of the cavoatrial tumor thrombus with an accuracy that appears equal to or better than that of any other method currently available. When used intraoperatively, it provides invaluable data to aid in the anesthetic and surgica… Show more

“…There are many publications on the preoperative and intraoperative use of TEE in patients with RCC invading the IVC. [12][13][14][15][16][17][18] There are several advantages of using TEE during the surgical management of RCC invading the IVC. In the present case, the anterior extent of the thrombus was accurately localized closer to the right atrium than, as detected by MRI.…”

“…Trendelenburg position resulted in migration of the thrombus away from TV, resulting in some improvement in hemodynamics, and given enough time to go on emergency CPB. The importance of intra-operative TEE in such surgeries is clearly supported[ 1 6 8 9 10 11 12 13 14 15 ] in medical literature and apart from helping in diagnosing thrombus embolization, TEE also helps in performing a comprehensive evaluation of residual thrombus if present after surgery. “…Renal tumors, such as nephroblastoma, clear cell sarcoma, 27,28 and renal cell carcinoma sometimes extend into the IVC, the hepatic vein, and the right side of the heart, because their drainage veins connect to the IVC. Several cases of intracaval or intracardiac extension of renal tumors have been reported, 12,[28][29][30][31][32][33][34] and renal tumors with intraatrial extension or hepatic vein obstruction (i.e., Budd-Chiari syndrome) may require emergent surgical treatment to avoid the fatal tricuspid obstruction, pulmonary embolization or acute hepatic failure. 28 Despite invasion of the IVC wall, our patient is free of disease 2 years after the operation.…”
