Purpose of reviewFunctional rhinoplasty is a term that is commonly used to describe any technique used to address nasal obstruction in the nasal valve region. Etiology of nasal valve collapse differs based on the site. Internal valve collapse may be idiopathic or associated with previous rhinoplasty, trauma, or weakened cartilage in older patients. External valve collapse is usually idiopathic and less likely to be associated with previous surgery. Various techniques have been developed over time, and the data in support of functional rhinoplasty continues to increase. Recent findingsThis article discusses popular techniques currently in use as well as newer techniques that have been described over the past several years. In addition, there is a trend toward utilizing objective testing and validated assessment scales to assess post-operative changes. SummaryFunctional rhinoplasty utilizes a multitude of techniques to address nasal valve collapse. While high-level data is lacking, the current literature is in support of nasal valve correction to improve nasal breathing. Most importantly, the technique employed is unique for each patient.

Alar retraction is one of the common problems seen in both primary and secondary rhinoplasty. Multiple techniques of varying complexity have been described to treat this common and challenging problem. (1,4) The presently used techniques are associated with scar and potential formation of crust. Thus, we attempted to overcome these shortcomings by using new grafting techniques to correct alar retraction. In this article, we describe the simplest way of correcting alar retraction by using an alar projection graft.

Introducción: La rinoplastía es una de las cirugías estéticas más desafiantes. Dentro de las alternativas quirúrgicas destaca el uso de diversos injertos e implantes los cuales debiesen ser conocidos por todos los cirujanos que realizan esta cirugía.Objetivo: Describir la experiencia del Departamento de Otorrinolaringología de la Pontificia Universidad Católica de Chile en el uso de injertos en rinoplastías.Material y método: Estudio retrospectivo descriptivo donde se revisaron protocolos operatorios de todos los pacientes operados en dicho comité entre marzo 2008 y agosto 2009.Resultados: Se realizaron 98 rinoseptoplastías por rinoseptodesviación en el período, siendo el 84% por abordaje abierto. El injerto más utilizado fue el vástago de columela en el 84% de todas las rinoplastías, seguido por el escudo anterior a la punta nasal (28%) y el espaciador entre laterales superiores y tabique (18%). Injertos alares, de dorso, de tabique y otros de refinamiento de punta fueron utilizados en menos del 5% de los casos. En todos los casos se utilizó injertos autólogos (tabique nasal). La gran mayoría de los injertos fue utilizada por abordaje abierto. Al momento de la revisión no hubo complicaciones que se puedan atribuir específicamente al uso de injertos.Discusión: Nuestra experiencia sigue la tendencia de "resecar menos" y rellenar. El uso de injertos resulta valioso dentro del repertorio quirúrgico en rinoplastía.Palabras clave: Rinoplastía, rinoseptoplastía, injertos.
