Use of Active Surveillance to Validate International Classification of Diseases Code Estimates of Rotavirus Hospitalizations in Children

Abstract: Measurement of rotavirus-coded hospital discharges alone seems to greatly underestimate the true burden of rotavirus-associated hospitalizations. The numbers of national rotavirus hospitalization discharges may be substantially greater than previously estimated.

“…Rotavirus is responsible for a substantial, yet underestimated, number of hospitalisations of children with acute gastroenteritis [17]. The incidence of rotavirus diarrhoea requiring hospital admission in children presenting at the emergency department was 40%, which is similar to that reported in other European countries [11,18,20,21], but lower than the 70% rate found in Canada [24].…”

“…We note that our estimate of 52% for the RV-fraction is similar to a recent US estimate (49%) obtained using direct linkage of hospital admissions and pathology data. 13 In addition to providing estimates of RV disease burden in young children in Australia, the study provided an opportunity to examine methodology for obtaining estimates of the RV fraction in light of data sources and admission coding practices. While an RV-specifi c ICD code (A08.0) has been available for some time, the number of AGE admissions for which a specifi c aetiology is not indicated in the patient record remains a large impediment to direct estimation of the RV fraction from routinely available data.…”

“…Thus, all children born in the region, regardless of their vaccination status, are included in LOGIVAC. 15 Both primary and secondary diagnoses were considered, and two hospitalizations for the same infant occurring in less than 14 days were considered as one event. Hospitalizations rates were calculated by dividing the number of hospitalizations that occurred among children aged less than five years by the total number of children of the same age group in the birth cohort over a specified period.…”
