Urodynamic variables cannot be used to classify the severity of detrusor instability

Abstract: Objective  To explore the relationship between subjective severity of symptoms of detrusor instability (DI) on presentation, outcome after treatment for DI and initial diagnostic urodynamic variables, with the aim of identifying a urodynamic variable which might, by predicting a favourable outcome from treatment, classify the severity of DI. Patients and methods  Women with a urodynamically proven diagnosis of DI were recruited prospectively for the study. Data on disease symptoms and variables from their diag… Show more

“…Although urodynamic investigation is considered an essential part of the assessment of urge incontinence, to enable a de¢nitive and objective diagnosis, the results often correlate poorly with patients' symptoms [Wiskind et al, 1994;Wagg et al, 1998]. As ¢lling cystometry can determine the presence or absence of unstable detrusor contractions, traditionally the management of urge incontinence has focused on detrusor instability.…”

“…It is commonly used in human patients for the treatment of detrusor instability [61-65] and its use has been reported in a dog and a cat [66]. Previous experimental studies in rat and dog models demonstrated an increased bladder volume with decreased bladder contraction pressure after high doses of oxybutynin [67], or an increased bladder volume without variation of bladder pressure [68,69].…”

“…-La présence d'une hyperactivité du détrusor sur l'examen urodynamique n'est pas prédictive des chances d'efficacité d'un traitement anticholinergique ou de la rééducation [26,27]. Cependant, la cystomanométrie, répétée en cas d'échec du traitement permettra d'évaluer les conséquences de ce traitement sur la contractilité vésicale.…”
