Urinary Proteome Analysis to Exclude Severe Vesicoureteral Reflux

Abstract: This noninvasive test is ready for prospective validation in large cohorts with the aim of identifying those children with UTI and hydronephrosis in need of further invasive diagnostics, such as voiding cystourethrography, thus sparing most children without pathologic urinary proteome patterns from additional diagnostics.

“…Because of its high negative predictive value, this analysis would be useful as a screening test to detect high‐grade VUR. Only children with a positive result would then require VCUG . Because the urinary proteome can include many urinary fibrosis‐related peptides from the extracellular matrix, it can act as a non‐invasive liquid biopsy to identify patients with CKD .…”

“…4547 The possibility that application of omics approaches to such samples could identify molecular signatures and prognostic biomarkers was suggested by findings from studies in common kidney disorders such as diabetic nephropathy, 48 allograft rejection, 49 and vesicoureteral reflux. 50 Changes in urinary miRNA profile have been detected in disorders such as lupus nephritis 51 and renal fibrosis. 52 The study of the urine metabolome by nuclear magnetic resonance spectroscopy and mass spectrometry is another emerging technology that can generate molecular fingerprints of diagnostic or prognostic value, 53 as already shown in patients with Fanconi’s syndrome.…”

“…In a small cohort of 73 children, Drube et al. developed a CE‐MS‐based urinary biomarker panel with high sensitivity (88%) and specificity (79%) for detecting VUR, which upon validation in a larger cohort, could spare young patients from going through invasive procedures to diagnose VUR .…”

“…High-grade vesicoureteral reflux (VUR) is a risk factor for impaired renal function and current diagnosis by voiding cystourethrography is highly invasive and only concerns a minority of children. In a small cohort of 73 children, Drube et al developed a CE-MS-based urinary biomarker panel with high sensitivity (88%) and specificity (79%) for detecting VUR, which upon validation in a larger cohort, could spare young patients from going through invasive procedures to diagnose VUR [96].…”
