Urinary bladder leiomyosarcoma in adults

Abstract: Leiomyosarcomas of the bladder have always been considered as a highly aggressive entity and little is known about their origin, clinicopathologic presentation, and the survival factors associated with them. Contemporary studies suggest that these tumors may have a better prognosis than once believed. Leiomyosarcomas require aggressive surgical extirpation and, when surgical resection is possible, radical cystectomy with wide margins is the rule and should be performed. Strict adherence to standard surgical te… Show more

“…Labanaris and colleagues reported a young female with a 1.2-cm tumour who was treated with partial surgery with negative surgical margins. 6 The 8-month follow-up revealed no local recurrence or distant metastasis. Tsujita and colleagues reported a huge bladder diverticulum leiomyosarcoma (>6 cm) with mesentery infiltration in a 68-year-old man who underwent partial cystectomy.…”

“…Most patients are already in the advanced pathologic stage when the tumour is detected; less than 15% of tumours are identified in the T1 stage. 6 Metastatic signs of the disease usually portend a dismal prognosis. Administration of neoadjuvant chemotherapy should be considered in cases of locally advanced or metastatic disease.…”

“…Leiomyosarcoma is the most common second malignancy in Hereditary Retinoblastoma. 1,2,3,4,5 Post radiation sarcomas can occur with orthovoltage (low-energy) and megavoltage (high-energy) radiation more than 40-60 Gy. Ionising radiation is thought to act via genetic alterations including mutations of p53 and Retinoblastoma (Rb) genes.…”

“…Sarcoma constitutes the most usual mesenchymal malignancy of the bladder, with leiomyosarcomas being the most common type of sarcoma in adults [5]. Small cell carcinomas of urinary bladder accounts for 0.35-0.70 % of all bladder tumors.…”
