Upper- vs lower-limb aerobic exercise rehabilitation in patients with symptomatic peripheral arterial disease: A randomized controlled trial

Abstract: Our results suggest that a combination of physiologic adaptations and improved exercise pain tolerance account for the improvement in walking performance achieved through upper-limb aerobic exercise training in patients with PAD. Furthermore, that both arm- and leg-crank training could be useful exercise training modalities for improving cardiovascular function, walking performance, and exercise pain tolerance in patients with symptomatic PAD.

“…Exercise programs reduce claudication symptoms and increase walking times (50,51). The improvements that are gained from exercise are thought to be related to improved angiogenesis and nitric oxide release, which improves endothelial dysfunction and exercise tolerance (52)(53)(54)).…”

“…Since this study a number of randomised controlled trials (Table I) have shown similar results. 8,19,22,23,25,29,30,35,36,41,44 It is clear from Table I that exercise training improves walking tolerance in patients with PVD. Indeed when comparing the effects of exercise training with PTA, exercise training was more beneficial after a 6-month period.…”

“…Polestriding, a stationary form of exercise similar to skiing has proved to be beneficial to patients with intermittent claudication, as has upper-limb cycle ergometry. 7,41,44 The effect of upperlimb and lower-limb cycle ergometry on walking tolerance in patients with intermittent claudication was assessed in the United Kingdom. 41,44 Patients were randomised to an upperlimb cycle ergometry group or a lower-limb cycle ergometry group and trained twice a week for 6 weeks.…”

“…Este mesmo autor aponta que o duplo produto é um excelente indicador para a prescrição de exercícios para populações que necessitam de cuidados cardíacos como é o caso dos idosos, o que sugere que seja utilizado de forma regular. É sabido que a proteção cardíaca do idoso passa por manter AF regular independentemente do gênero (ZWIERSKA et al, 2005).…”
