Unusual presentation of Bouveret's syndrome

Abstract: Isolated gastric outlet and ileal obstruction are rare complications of gallstones. We report what we believe to be the first case of a gallstone causing gastric outlet obstruction while eroding into the duodenum, yet once in the duodenum, passing into the small bowel, causing ileal obstruction. The endoscopic appearance of the eroding gallstone was so unusual that it was initially mistaken for a fibrotic duodenal ulcer.

“…The first endoscopic diagnosis was made by Grove in 1976 [44], with endoscopic retrieval first being achieved by Bedogni et al in 1985 [45]. Endoscopically, the appearances can be bizarre, with fibrotic-looking ulcers that subsequently turn out to be a fistulating gallstone [14]. This is in keeping with the endoscopic findings in the case presented here.…”

“…Due to the nearby inflammation, the gallbladder can be FDG/PET positive [178] . Spontaneous resolution can occur when the impacted stone falls back away from the pyloric orifice [16] , but this can be associated with further bowel obstruction distal to the stomach and duodenum (gall stone ileus) [114,141] . On the other hand, the condition can be fatal due to the profound metabolic derangement [13] , and later by sepsis and multi-organ failure [62] .…”
