Unusual Annular Erythema Associated with Myelodysplastic Syndrome

Abstract: We report a case of unusual annular erythema associated with myelodysplastic syndrome (MDS). A 58-year-old male with MDS developed annular erythema on his back, scaly erythema on the dorsa of hands, and exsudative erythema on his eyelids. Histological examination revealed a mononuclear cell infiltrate around vessels and follicles in the mid- to lower dermis. He had no history of treatment with granulocyte-colony-stimulating factor (G-CSF). Serum granulocyte-macrophage-colony-stimulating factor (GM-CSF) level w… Show more

“…H-SS was previously shown to occur at the onset or preceding MDS in most cases. 10 , 39 , 40 In our study, the hematological disease was mostly diagnosed before the diagnosis of H-SS. However, 3 patients presented cutaneous lesions of H-SS before the diagnosis of hematological malignancy, MDS in all these cases, within a 6-month interval.…”

“…13,18,19 Myelodysplastic syndrome occurred at the onset of cutaneous disease in 2 patients or followed it 3 years later in 1. In a series of 37 patients with Sweet syndrome who had undergone sequential biopsies, the initial lymphocytic stage was followed by neutrophilic and then histiocytic infiltrates.…”

“…18 There is a report of annular erythema in a patient with myelodysplasia clinically resembling SS but with a mononuclearpredominant dermal infiltrate. 19 One of us (personal observation, J.C.M., 2003) has observed interstitial granulomatous dermatitis with small mononuclear cells presumed to be monocytes or small histiocytes in a mixed infiltrate containing many neutrophils and eosinophils. The true origin of these cells is a question begging to be answered with a battery of immunohistochemical, cytochemical, and enzymatic stains.…”
