BackgroundSuccessful extraction of third molars depends on preoperative diagnosis and planning. Gold standard preoperative examinations are performed through computed tomography, decreasing risks and avoiding potential accidents. The present report highlights the value of preoperative examinations in face of accidentally displaced third molars.MethodsAn 18-years-old female patient underwent a third mandibular molar extraction with a general dentist. Accidentally, the mandibular left third molar was displaced into the sublingual space, making necessary a second surgical step. The surgery was interrupted and the patient was referred to an expert in maxillofacial surgery.ResultsAfter 21 days awaiting an asymptomatic health status, the second surgical step was successfully performed using multislice computed tomography as preoperative imaging guide.ConclusionsThe present case report highlights the clinical usefulness of imaging planning and informed consents in face of legal and ethic potential complaints.

Résumé -Introduction :Le déplacement ou le refoulement accidentel de la dent de sagesse dans les espaces de voisinages est une complication rare mais possible lors de son avulsion. Observation : Deux cas de déplacement accidentel de troisièmes molaires mandibulaires dans l'espace sub-mandibulaire sont rapportés. Le diagnostic topographique, la conduite du traitement et les causes de cette complication seront discutés. Discussion : Le déplacement peut intéresser l'espace sublingual, l'espace ptérygo-mandibulaire et l'espace sub-mandibulaire pour les 3 es molaires mandibulaires, les espaces cellulaires, le sinus maxillaire et la fosse ptérygo-maxillaire pour les 3 es molaires maxillaires. L'absence d'évaluation pré-chirurgicale des facteurs de risque a été citée comme la cause la plus fréquente de ces accidents notamment au niveau mandibulaire. Une bonne planification préopératoire, une analyse radiographique minutieuse et la maîtrise de la technique chirurgicale sont d'une importance capitale pour diminuer l'incidence de ces complications.Abstract -Accidental displacement of third molar into the submandibular space: two case reports. Introduction: The accidental displacement of the wisdom tooth into nearby spaces is a rare, but possible complication during extraction. Case report: We report two cases of accidental displacement of mandibular third molars into the submandibular space. Topographic diagnosis, treatment and the causes of this complication are discussed. Discussion: Displacement may involve the sublingual space, pterygomandibular space and submandibular space for the third lower molars, and cellular spaces, maxillary sinus and pterygomaxillary fossa for the third upper molars. The most frequent cause cited for these accidents, particularly for mandibular molars, was the lack of presurgery assessment of risk factors. Good preoperative planning, careful radiographic evaluation, and mastery of the surgical technique are of crucial importance in reducing the incidence of these complications.

Now you see it, now you don't … A case of a displaced root during a routine dental extraction

Routine dental extractions are commonly carried out in general dental practice. Complications associated with extractions in the maxillary arch are rare but well recognised such as oral antral communications and fracture of the buccal plate. In this article, we describe an unusual complication of a ‘disappearing’ canine root and the difficulties in locating its position. To examine the different image modalities employed to determine the location of the root and the advantages of three‐dimensional assessment with cone beam computed tomography (CBCT) in such cases. In this case, the use of CBCT was invaluable, as the missing root was not initially located using plain film radiographs, but was subsequently located with the use of CBCT. The inadequacy of the plain film and its interpretation in this case caused delay in the patient receiving the required treatment.
