Unilateral multicystic dysplastic kidney in infants exposed to antiepileptic drugs during pregnancy

Abstract: Prenatal exposure to antiepileptic drugs (AEDs) increases the risk of major congenital malformations (MCM) in the fetus. AED-related abnormalities include heart and neural tube defects, cleft palate, and urogenital abnormalities. Among the various congenital anomalies of the kidney and urinary tract (CAKUT), multicystic dysplastic kidney (MCDK) disease is one of the most severe expressions. Although prenatal ultrasound (US) examination has increased the prenatal diagnosis of MCDK, the pathogenesis is still unc… Show more

“…Another report documented a terminated pregnancy in which the fetus had a unilateral MCDK, with CMV inclusions in the tubular epithelium, and a normal contralateral kidney without inclusions [15]. A series of four infants with MCDKs and epileptic mothers treated during pregnancy with antiepileptic drugs raises the possibility that medications may be a contributing factor [16]. Other reports, however, suggest that exposure to teratogens is rarely identified in patients with MCDKs [17].…”

“…Fourth, antiepileptic drugs during pregnancy are known teratogens (43) and have been described to increase the risk of congenital malformations in general and of multicystic dysplastic kidney specifically (44). This may be due to increased levels of the B cell lymphoma 2 protein, leading to defective apoptosis and thereby defective nephrogenesis (44).…”

“…In one study, 1-3% of children with congenital kidney malformations had amniotic fluid that produced cultures positive for enterovirus, cytomegalovirus or adenovirus (14). A series of four infants with MCDK and epileptic mothers treated during pregnancy with antiepileptic drugs raises the possibility that medications may be a contributing factor (15).…”
