Unilateral acute idiopathic maculopathy secondary to yellow fever disease: a case report

Abstract: Purpose To report a case of unilateral acute idiopathic maculopathy (UAIM) associated with yellow fever. Observations A 59-year-old man presented with acute blurring of his vision 30 days after symptoms of yellow fever virus infection. Findings resembling unilateral acute idiopathic maculopathy of the left eye were noted on ophthalmoscopy, fluorescein angiography and optical coherence tomography. The right eye exam was normal. He was managed conservatively and recovered… Show more

“…In our case, the outer retinal and RPE anatomic changes with subretinal fluid and disrupted ellipsoid zone of the photoreceptors following the recovery of the anatomic changes and the visual acuity are consistent with AIM. 1 , 2 , 3 , 4 , 8 , 9 , 10 , 11 However, the thickening of the RPE and persistent RPE defect were more severe than that reported for AIMs. 1 , 2 , 3 , 4 , 8 , 9 , 10 , 11 With the Pfizer vaccines, 13 the mRNA for a spike protein is encapsulated in lipid nanoparticles which are endocytosed into muscle cells.…”

“…In the literature, the state of health at the time of UAIM or just prior to the ophthalmic manifestation has been clearly documented in 46 cases, with the following findings: flulike illness (n = 13), 6 , 21 , 22 , 23 , 24 , 25 , 26 healthy (negative for recent medical history; n = 12), 6 , 9 , 12 , 14 , 19 , 27 , 28 , 29 , 30 , 31 , 32 HFMD (n = 7), 8 , 9 , 20 , 33 , 34 , 35 pregnancy (n = 3), 1 , 7 orchitis (n = 2), 9 anterior uveitis (n = 1), 12 acute tonsillitis (n = 1), 12 pharyngitis (n = 1), 18 laryngitis (n = 1), 15 gastroenteritis (n = 1), 13 and Hashimoto disease (n = 1). 12 Other specific viral infections have also been implicated, including HIV (n = 1), 7 yellow fever (n = 1), 36 and Zika virus (n = 1). 37 In 26 cases (57%) it seems that a viral infection was coincident with disease, which is not readily understandable, given the far more common unilateral presentation of UAIM when bilateral disease would be expected.…”

“…Less commonly HIV, flavivirus febricis infection, and H1N1 vaccination were reported to be associated as well. [3][4][5][6] The prognosis is generally good, with poor vision usually resulting from complications such as choroidal neovascularization or subfoveal pigment degeneration. 1,7 Here, we report multimodal imaging findings in a patient with acute idiopathic maculopathy.…”
